甲地孕酮治疗恶病质的疗效比较——基于网状Meta分析的结果

摘　　要：目的系统评价不同代际甲地孕酮治疗恶病质患者的疗效。方法计算机检索PubMed和中国期刊全文数据库,搜集有关甲地孕酮治疗恶病质的随机对照临床试验,检索时限均为建库至2024年6月。两位研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用R3.6.1软件进行贝叶斯网状Meta分析。结果共纳入3个研究,针对疗效指标终点-基线体重恢复≥2.3 kg的患者比例的网状Meta分析结果显示:与醋酸甲地孕酮分散片相比,纳米晶型高浓度醋酸甲地孕酮口服混悬液能显著提高患者体重恢复(≥2.3 kg)的比例[RR=2.6,95%CI(0.90,8.5)],实现体重明显恢复的患者比例更高(60%vs.23%)。结论纳米晶型高浓度醋酸甲地孕酮口服混悬液在治疗恶病质的临床有效性方面比目前普遍使用的分散片具有一定优势。Objective To systematically evaluate the efficacy of different generations of megestrol in the treatment of patients with cachexia.Methods Computer searched PubMed and CNKI databases to collect randomized controlled studies of different generations of megestrol in the treatment of patients with cachexia.The search time was from the establishment of the database to June 2024.Two reviewers independently screened the literature,extracted data,and evaluated the risk of bias in the included studies.R software was used for Bayesian network meta-analysis.Results A total of 3 studies were enrolled.Network meta-analysis results for the efficacy indicator showed that:compared with megestrol acetate dispersible tablets(800 mg/day),the proportion of patients increasing body weight(≥2.3 kg)was significantly higher in the nanocrystal enhanced suspension group[RR=2.6,95%CI(0.90,8.5)],with a proportion of 23%vs.60%,respectively.Conclusion Existing evidence indicates that nanocrystal concentrated megestrol suspension has better efficacy on increasing patients'body weight over the dispersible tablets.
