奥卡西平联合左乙拉西坦治疗癫痫对患儿认知功能及载脂蛋白的影响

摘　　要：目的探讨奥卡西平联合左乙拉西坦治疗癫痫对患儿认知能力及载脂蛋白的影响。方法选取2020年1月至2023年12月在漯河医学高等专科学校第二附属医院儿科治疗的78例癫痫患儿,按治疗方法分组,每组26例,即奥卡西平组给予奥卡西平口服混悬液治疗,左乙拉西坦组给予左乙拉西坦口服溶液治疗,联合组给予奥卡西平口服悬液和左乙拉西坦口服溶液治疗。比较3组患儿临床疗效、不良反应发生情况、治疗前后认知功能评分、血清载脂蛋白水平。结果与单药组比较,联合组治疗总有效率较高(P<0.05)。与单药组比较,联合组不良反应发生率较低(P<0.05)。治疗后3组操作智商(PIQ)评分、言语智商(VIQ)评分以及总智商(FIQ)评分升高(P<0.05),且联合组高于单药组(P<0.05);3组血清载脂蛋白A1降低,载脂蛋白B、B/A1比值升高,联合组优于单药组,差异有统计学意义(P<0.05)。结论奥卡西平与左乙拉西坦联合治疗癫痫患儿具有显著疗效,能提高认知功能,降低不良反应发生率,改善患儿生命质量,降低未来患心脑血管疾病的风险。Objective To explore the effects of oxcarbazepine combined with levetiracetam on cognitive ability and apolipoprotein in children with epilepsy.Methods A total of 78 children with epilepsy who were treated in the Department of Pediatrics of the Second Affiliated Hospital of Luohe Medical College from January 2020 to December 2023 were selected and divided into 26 cases in each group according to the admission treatment method,that is,oxcarbazepine group was given oral suspension treatment with oxcarbazepine,and levetiracetam group was given oral solution treatment with levetiracetam.The combined group was treated with oxcarbamazepine oral suspension and levetiracetam oral solution.The clinical efficacy,occurrence of adverse reactions,cognitive function score,and serum apolipoprotein level before and after treatment of the 3 groups were compared.Results Compared with monotherapy group,the total effective rate of combination group was higher(P<0.05).Compared with monotherapy group,the incidence of adverse reactions in combination group was lower(P<0.05).After treatment,the operative intelligence quotient(PIQ),verbal Intelligence quotient(VIQ)and total Intelligence quotient(FIQ)scores of the 3 groups were increased(P<0.05),and the combined group was higher than the monotherapy group(P<0.05).Serum apolipoprotein A1 was decreased in 3 groups,and apolipoprotein B and B/A1 ratio were increased,combination group were better than monotherapy group,the difference was statistically significant(P<0.05).Conclusion Oxcarbazepine combined with levetiracetam is effective in the treatment of pediatric epilepsy,can improve cognitive function,and has a low incidence of adverse reactions,improves the quality of life of children,and reduces the risk of cardiovascular and cerebrovascular diseases in the future.
