注射用血栓通联合补阳还五汤治疗急性脑梗死的效果观察

摘　　要：目的 探讨注射用血栓通联合补阳还五汤治疗急性脑梗死临床疗效。方法 以简单随机抽样法选取贵州省金沙县人民医院急性脑梗死患者100例作为研究对象,收集时间为2022年7月至2024年7月,根据随机数字表法划分两组。对照组50例,应用注射用血栓通,观察组50例,应用注射用血栓通联合补阳还五汤。比较两组患者神经功能、预后、日常生活能力、脑血流量、临床疗效、病灶侧支循环血管分级、不良反应发生率。结果治疗后,观察组NIHSS评分、mRS评分均低于对照组(P<0.05),BI评分、大脑前、中、后动脉收缩期最高血流速度、总有效率均高于对照组(P<0.05),病灶侧支循环血管分级0级比例低于对照组,1级、2级比例均高于对照组(P<0.05)。结论 急性脑梗死治疗中注射用血栓通联合补阳还五汤治疗的效果较单独用血栓通好,更能增加患者脑血流量,提升病灶侧支循环血管分级,改善患者神经功能、预后,提升患者日常生活能力,临床疗效提升,减少不良反应的发生。Objective To investigate the clinical efficacy of intravenous thrombolytic therapy with Xuefuzhuyu injection combined with Buyang Huawu Decoction in the treatment of acute cerebral infarction.Methods This study employed a simple random sampling method to select 100 patients diagnosed with acute cerebral infarction from the People's Hospital of Jinhua County,Guizhou Province,from July 2022 to July 2024.Patients were randomly assigned to two groups using a random number table:the control group(n=50)received intravenous thrombolytic therapy with Xuefuzhuyu injection,while the observation group(n=50)received Xuefuzhuyu injection in conjunction with Buyang Huawu Decoction.Statistical analysis was performed to compare neurological function,prognosis,activities of daily living,cerebral blood flow,clinical efficacy,collateral circulation grading,and incidence of adverse effects between the two groups.Results After treatment,the observation group exhibited significantly lower NIHSS and mRS scores compared to the control group(P<0.05).Additionally,the observation group demonstrated higher Barthel Index(BI)scores,peak systolic blood flow velocities in the anterior,middle,and posterior cerebral arteries,and a greater overall effective rate(P<0.05).The proportion of collateral circulation grading level 0 in the observation group was lower than that in the control group,while the proportions of levels 1 and 2 were higher(P<0.05).Conclusion The combination of intravenous thrombolytic therapy with Xuefuzhuyu injection and Buyang Huawu Decoction exhibits superior effectiveness in the treatment of acute cerebral infarction compared to Xuefuzhuyu injection alone.This combined approach significantly enhances cerebral blood flow,improves collateral circulation grading,benefits neurological function and prognosis,enhances patients'activities of daily living,and reduces the incidence of adverse effects.
