低频重复经颅磁刺激对青少年抑郁症患者自杀意念影响的实证研究

摘　　要：目的本研究旨在评估低频重复经颅磁刺激(rTMS)如何改善青少年抑郁症患者的抑郁症状与自杀意念,为治疗该群体提供新颖思路与方案。方法选取2022年5月~2024年5月期间在贵州医科大学第二附属医院精神科诊治的84例青少年抑郁症患者,采用随机数字表法将患者分为两组,即研究组和对照组,每组42例。对照组仅接受盐酸氟西汀治疗,研究组在此基础上接受低频rTMS治疗。比较两组患者血清生化指标、血清miRNA水平、心理与睡眠状况、自杀意念的严重程度及不良反应。计数资料以x2检验,正态计量资料以t检验。结果治疗前,两组生化指标、miRNA水平、心理和睡眠状况、自杀意念的严重程度及不良反应无显著差异(P>0.05);治疗后,研究组血清生化指标显著高于对照组,血清miRNA水平、心理和睡眠状况、自杀意念、自杀倾向、总分均显著低于对照组(P<0.05)。研究组不良反应总发生率低于对照组(P<0.05)。结论低频rTMS是一种安全有效的疗法,能明显减轻青少年抑郁症患者的症状,减少自杀念头,提升其生活质量。Objective The aim of this study is to evaluate how low-frequency repetitive transcranial magnetic stimulation can improve depressive symptoms and suicidal ideation in adolescent patients with depression,providing novel ideas and strategies for the treatment of this population.Methods 84 adolescent depression patients treated in the Department of Psychiatry of the Second Affiliated Hospital of Guizhou Medical University from May 2022 to May 2024 were selected.The patients were divided into two groups using a digital random table method,namely the study group and the control group,with 42 patients in each group.The control group received only fluoxetine hydrochloride treatment,while the study group received low-frequency rTMS treatment on this basis.Compare the serum biochemical indicators,serum miRNA levels,psychological and sleep status,severity of suicidal ideation,and adverse reactions between two groups of patients.Count data is tested by x2,and normal measurement data is tested by t-test.Results Before treatment,there were no significant differences in biochemical indicators,miRNA levels,psychological and sleep status,severity of suicidal ideation,and adverse reactions between the two groups(P>0.05);after treatment,the biochemical indicators of the research group were significantly higher than those of the control group;the serum miRNA levels,psychological and sleep conditions,suicidal ideation,suicidal tendencies and total scores were significantly lower than those of the control group(P<0.05).The total incidence of adverse reactions in the research group was lower than that in the control group(P<0.05).Conclusion Low-frequency rTMS is a safe and effective therapy that can significantly alleviate symptoms of depression in adolescents,reduce suicidal thoughts,and improve their quality of life.
