摘　　要：目的探讨血清miR-7-5p和miR-200b-3p在中重度大动脉粥样硬化性狭窄的缺血性脑卒中(CIS)的表达水平及对临床的预测价值。方法选择2021年5月至2023年5月在乌鲁木齐市友谊医院神经内科住院治疗且经脑血管造影术(DSA)证实有中、重度动脉粥样硬化性脑血管狭窄的CIS患者83例为病例组,同期取90例健康体检者作为对照组,对照组患者均进行了头颈部电子计算机断层扫描血管成像(CTA)检查,均不合并大动脉粥样硬化性狭窄。应用荧光定量反转录-聚合酶链式反应(RT-PCR)方法检测病例组和对照组miR-7-5p和miR-200b-3p的表达水平。分析两组基线资料、生化指标及miR-7-5p和miR-200b-3p的表达水平,采用受试者工作特征曲线(ROC)分析miR-7-5p和miR-200b-3p对中重度大动脉粥样硬化性狭窄的脑卒中的预测价值。结果(1)病例组患者有吸烟、饮酒不良嗜好及合并有糖尿病、高血压病史的人群比率高于对照组,差异有统计学意义(P<0.05),两组患者年龄、性别、入院时心率、血压比较,差异无统计学意义(P>0.05)。(2)病例组高密度脂蛋白胆固醇(HDL-C)低于对照组,肌酐(Cr)、FPG、HbAlc、Hcy、miR-7-5p、miR-200b-3p均高于对照组,差异有统计学意义(P<0.05),两组其余变量比较,差异无统计学意义(P>0.05)。(3)糖尿病(OR=3.892,95%CI 1.803~8.401)、高血压(OR=2.104,95%CI 1.036~4.273)、Cr(OR=1.027,95%CI 1.006~1.049)、HDL-C(OR=0.182,95%CI 0.036~0.920)、miR-7-5p(OR=1.960,95%CI 1.290~2.977)、miR-200b-3p(OR=2.980,95%CI 1.596~5.562)为中重度大动脉粥样硬化性狭窄的CIS的影响因素,差异均有统计学意义(P<0.05)。(4)受试者工作特征(ROC)曲线显示miR-7-5p的曲线下面积(AUC)、敏感度、特异度、约登指数分别为0.758、69.90%、72.20%、0.421;miR-200b-3p的AUC、敏感度、特异度、约登指数分别为0.798、75.90%、75.60%、0.515。提示miR-7-5p、miR-200b-3p对中重度大动脉粥样硬化性狭窄的CIS的诊断有一定的�Objective To investigate the expression levels of serum miR-7-5p and miR-200b-3p in moderate to severe atherosclerotic stenosis cerebral ischemic stroke(CIS)and their predictive clinical value.Methods From May 2021 to May 2023,83 CIS patients with moderate to severe atherosclerotic stenosis,confirmed by cerebral angiography(DSA)and treated at the Department of Neurology,Urumqi Friendship Hospital,were selected as the case group.Simultaneously,90 healthy individuals,confirmed through head and neck computed tomography angiography(CTA),were selected as the control group and none of them was associated with atherosclerotic stenosis.The expression levels of miR-7-5p and miR-200b-3p were measured using fluorescent quantitative reverse transcription-polymerase chain reaction(RT-PCR).Baseline data,biochemical indicators,and expression levels of miR-7-5p and miR-200b-3p of the two groups were analyzed,and receiver operating characteristic curves(ROC)were used to analyze the predictive value of these miRNAs for moderate to severe atherosclerotic stroke.Results(1)The proportion of patients with smoking and alcohol consumption habits,along with diabetes and hypertension history,was higher in the case group than in the control group,showing a statistically significant difference(P<0.05).There was no statistically significant difference between the groups in terms of age,gender,heart rate,or blood pressure upon admission(P>0.05).(2)In the case group,high-density lipoprotein cholesterol(HDL-C)was lower,while creatinine(Cr),fasting plasma glucose(FPG),HbA1c,homocysteine(Hcy),miR-7-5p,and miR-200b-3p were higher than those in the control group,with statistically significant differences(P<0.05).No statistical significance was found in other variables between the groups(P>0.05).(3)Diabetes(OR=3.892,95%CI 1.803~8.401),hypertension(OR=2.104,95%CI 1.036~4.273),Cr(OR=1.027,95%CI 1.006~1.049),HDL-C(OR=0.182,95%CI 0.036~0.920),miR-7-5p(OR=1.960,95%CI 1.290~2.977),and miR-200b-3p(OR=2.980,95%CI 1.596~5.562)were independent risk factors fo
