羊膜移植术与自体结膜瓣移植术治疗翼状胬肉的临床疗效对比分析

摘　　要：目的探究翼状胬肉的临床治疗期间,两种不同手术方案(羊膜移植术、自体结膜瓣移植术)的应用价值。方法选取苏州工业园区星湖医院2020年1月至2023年12月期间,收入的翼状胬肉病例80例,依据手术方案不同将其分为研究组(n=40,自体结膜瓣移植术)和对照组(n=40,羊膜移植术),分析其在临床恢复情况、疼痛程度以及角膜与翼状胬肉情况方面的差异。结果临床恢复情况方面,两组眼角膜上皮愈合程度评分差异无统计学意义(P>0.05),但在复发情况方面,研究组(2.50%)明显低于对照组(15.00%),差异具有统计学意义(χ^(2)=3.914、P=0.048);疼痛评分方面,研究组术后1 d、3 d、5 d、7 d的评分均低于对照组,差异具有统计学意义(P<0.05);角膜与翼状胬肉情况方面,术前各参数指标差异均无统计学意义(P>0.05),但术后30 d,研究组中央光学区的角膜曲率高于对照组,平均角膜表面形状因子、曲率角膜顶点与光心的距离、角膜不规则性均低于对照组,差异具有统计学意义(P<0.05)。结论翼状胬肉的临床治疗期间,两种不同手术方案(羊膜移植术、自体结膜瓣移植术)的应用价值均较为理想,但自体结膜瓣移植术的复发率以及术后疼痛情况更优,具有临床应用意义。Objective To explore the clinical value of two different surgical approaches(amniotic membrane grafting and autologous conjunctival flap grafting)during the treatment of pterygium.Methods A total of 80 cases of pterygium treated at Xinghu Hospital from January 2020 to December 2023 were selected.Based on the surgical method,patients were divided into the study group(n=40,autologous conjunctival flap grafting)and the control group(n=40,amniotic membrane grafting).The differences in clinical recovery,pain levels,and conditions of the cornea and pterygium were analyzed.Results In terms of clinical recovery,there was no statistically significant difference in corneal epithelial healing scores between the two groups(P>0.05).However,the recurrence rate in the study group(2.50%)was significantly lower than that in the control group(15.00%),with statistical significance(χ²=3.914,P=0.048).Regarding pain scores,the scores at 1,3,5,and 7 days post-surgery in the study group were consistently lower than those in the control group,showing significant differences(P<0.05).For the cornea and pterygium conditions,there were no significant differences in preoperative parameters between the groups(P>0.05).However,30 days post-surgery,the corneal curvature in the central optical zone of the study group was higher than that of the control group,while the average corneal surface shape factor,the distance from the apex of the cornea to the optical center,and corneal irregularity were lower in the study group,with statistical significance(P<0.05).Conclusion Both surgical methods(amniotic membrane grafting and autologous conjunctival flap grafting)show ideal clinical value in the treatment of pterygium.However,autologous conjunctival flap grafting has a lower recurrence rate and reduced postoperative pain,indicating significant clinical application value.
