摘  要:目的研究卵巢癌(OVC)组织中miR-92a、NOC2类核仁相关转录阻遏物(NOC2L)表达及其与临床病理特征和预后的关系。方法选取2019年1月-2021年12月武汉市第三医院妇产科诊治的OVC患者103例为研究对象。采用实时荧光定量PCR检测癌组织和癌旁组织miR-92a、NOC2L mRNA表达;比较不同临床病理特征中miR-92a、NOC2L表达差异并分析其相关性;受试者工作特征(ROC)曲线分析癌组织中miR-92a、NOC2L mRNA表达对OVC患者预后死亡的预测效能并采用DeLong法比较其差异;采用多因素Logistic回归分析OVC患者死亡的影响因素;应用K-M法及Log Rank比较不同miR-92a、NOC2L表达中生存期的差异。结果OVC患者癌组织中miR-92a、NOC2L mRNA的相对表达量高于癌旁组织(t/P=22.995/<0.001、16.691/<0.001);Shimizu-Silverberg分级3级、腹水细胞学阳性、淋巴结转移N1、远处转移M1、FIGO分期Ⅲ~Ⅳ期的OVC患者癌组织中miR-92a、NOC2L mRNA表达高于Shimizu-Silverberg分级1~2级、腹水细胞学阴性、淋巴结转移N0、远处转移M0、FIGO分期Ⅰ~Ⅱ期(miR-92a:t/P=2.020/0.046、2.635/0.009、1.994/0.049、2.981/0.004、2.807/0.006;NOC2L:t/P=3.483/0.001、3.729/<0.001、3.183/0.002、6.269/<0.001、4.318/<0.001);OVC患者癌组织中miR-92a、NOC2L mRNA表达分别与Shimizu-Silverberg分级、腹水细胞学、淋巴结转移、远处转移、FIGO分期呈正相关(miR-92a:r s/P=0.678/0.024、0.715/0.007、0.699/0.015、0.742/0.003、0.687/0.029;NOC2L:r s/P=0.631/0.015、0.687/0.029、0.711/0.024、0.659/0.031、0.702/0.008);miR-92a、NOC2L及二者联合预测OVC患者死亡的曲线下面积(AUC)分别为0.684、0.701、0.896,二者联合优于各自单独预测效能(Z/P=7.145/<0.001、6.097/<0.001);多因素Logistic回归分析显示,Shimizu-Silverberg分级3级、腹水细胞学阳性、淋巴结转移N1、远处转移M1、FIGO分期Ⅲ~Ⅳ期、miR-92a≥1.45、NOC2L≥0.86为OVC患者预后死亡的独立危险因素[OR(95%CI)=1.602(1.045~2.158)、2.032(1.144~2.921Objective To study the expression of miR-92a and NOC2-like nucleolar-associated transcriptional repressor(NOC2L)in ovarian cancer(OVC)tissues and their relationship with clinicopathological characteristics and prognosis.Methods A total of 103 OVC patients treated in Wuhan Third Hospital from January 2019 to December 2021 were selected as the research subjects.Real-time fluorescence quantitative PCR was used to detect the mRNA expressions of miR-92a and NOC2L in cancer tissues and adjacent tissues.Receiver operating characteristic(ROC)curve was used to analyze the predictive efficacy of miR-92a and NOC2L mRNA expressions in cancer tissues on the prognosis and death of OVC patients,and the DeLong method was used to compare the differences.Multivariate Logistic regression was used to analyze the influencing factors of the prognosis and death of OVC patients.The K-M method and Log Rank method were used to compare the differences in survival time in different miR-92a and NOC2L expressions.Results The relative expression levels of miR-92a and NOC2L mRNA in cancer tissues of OVC patients were higher than those in adjacent tissues(t/P=22.995/<0.001,16.691/<0.001).The mRNA expressions of miR-92a and NOC2L in cancer tissues of OVC patients with Shimizu-Silverberg grade 3,positive ascites cytology,lymph node metastasis N1,distant metastasis M1,and FIGO stageⅢ-Ⅳwere higher than those with Shimizu-Silverberg grade 1-2,negative ascites cytology,lymph node metastasis N0,distant metastasis M0,and FIGO stageⅠ-Ⅱ(miR-92a:t/P=2.020/0.046、2.635/0.009、1.994/0.049、2.981/0.004、2.807/0.006;NOC2L:t/P=3.483/0.001、3.729/<0.001、3.183/0.002、6.269/<0.001、4.318/<0.001).The mRNA expression was positively correlated with Shimizu-Silverberg grade,ascites cytology,lymph node metastasis,distant metastasis,and FIGO stage(miR-92a:r s/P=0.678/0.024,0.715/0.007,0.699/0.015,0.742/0.003,0.687/0.029;NOC2L:r s/P=0.631/0.015,0.687/0.029,0.711/0.024,0.659/0.031,0.702/0.008).The areas under the curve(AUC)of miR-92a,NOC2L and their combi
