干燥综合征患者外周血中Th17/Treg及Breg水平与临床意义

摘　　要：目的:检测原发性干燥综合征(Primary Sjogren's syndrome,pSS)患者外周血中辅助性T细胞17(T helper 17 cells,Th17)/调节性T细胞(Regulatory T cells,Treg)比率及调节性B细胞(B regulatory cells,Breg细胞)水平及临床意义。方法:选取2023年1月至2024年1月期间于本院就诊的原发性干燥综合征患者30例作为试验组。同时选取于本院进行体检的30例健康体检者为对照组。试验组治疗6m。采集试验组与对照组治疗前后的外周血,分别检测外周血红细胞沉降率(Erythrocyte Sedimentation Rate,ESR)、C反应蛋白(C-Reactive Protein,CRP)以及免疫球蛋白G(Immunoglobulin G,IgG)。进行欧洲干燥综合征疾病活动指数(European League Against Rheumatism Sjögren's Syndrome Disease Activity Index,ESSDAI)评分。分析对比两组外周血中Th17、Treg与Breg以及Th17/Treg。分析对比试验组治疗前后的ESSDAI评分、Th17、Treg与Breg以及Th17/Treg。分析试验组外周血Th17/Treg和Breg与患者的ESSDAI评分及血清ESR、CRP、IgG水平的相关性。结果:试验组患者治疗前的血清ESR、CRP和IgG水平均明显高于对照组(P<0.05),外周血Breg、Treg比例低于对照组(P<0.05),Th17比例、Th17/Treg均明显高于对照组(P<0.05)。与治疗前相比,治疗后试验组患者的ESSDAI评分、ESR、CRP和IgG水平均明显降低(P<0.05),外周血Breg与Treg比例较治疗前无显著差异;外周血中Th17比例、Th17/Treg降低(P<0.05)。Spearman相关性分析结果显示,试验组治疗前外周血中Th17/Treg细胞比率与ESSDAI评分、血清ESR水平呈正相关(P<0.05)。结论:pSS患者体内存在Th17/Treg细胞比例失衡以及Breg细胞水平的异常。Objective:Assessment of the ratio of T helper 17 cells(Th17 cells)to Regulatory T cells(Treg cells)and the level of B regulatory cells(Breg cells)in the peripheral blood of patients with primary Sjögren's syndrome(pSS),along with their clinical significance.Methods:Thirty patients with pSS who visited our hospital between January 2023 and January 2024 were selected as the experimental group.Simultaneously,thirty healthy individuals underwent physical examinations in our hospital were selected as the control group.The experimental group received treatment for six months.The Erythrocyte Sedimentation Rate(ESR),C-Reactive Protein(CRP),and Immunoglobulin G(IgG)were compared between the two groups as well as before and after treatment of experimental group.The European League Against Rheumatism Sjögren's Syndrome Disease Activity Index(ESSDAI)was assessed in experimental group.Th17,Treg and Breg and Th17/Treg in peripheral blood of the two groups were analyzed and compared.ESSDAI score,Th17,Treg and Breg,and Th17/Treg before and after treatment were analyzed and compared.Correlation analyses were performed between the Th17/Treg ratio and Breg levels in the peripheral blood of the experimental group and their ESSDAI scores,as well as serum levels of ESR,CRP,and IgG.Results:The serum ESR,CRP and IgG levels of patients in the experimental group before treatment were significantly higher than those in the control group(P<0.05).The proportions of Breg and Treg in peripheral blood were significantly lower than those in the control group(P<0.05).The proportion of Th17 and Th17/Treg were significantly higher than those in the control group(P<0.05).Compared with before treatment,the ESSDAI score,serum ESR,CRP and IgG levels of patients in the experimental group were significantly decreased after treatment(P<0.05).There was no significant difference in the proportion of Breg and Treg in peripheral blood compared with before treatment;the proportion of Th17 in peripheral blood,Th17/Treg was significantly reduced(P<0.05).Spearm
