摘　　要：目的:对比Protaper机械镍钛锉与手持不锈钢K锉在根尖周病变患者根管治疗中的应用效果。方法:采用回顾性研究分析,根据根管预备器械的不同将2023年1月至2024年9月本院收治的根尖周病变患者(60例,患牙65颗)分为A组(使用Protaper机械镍钛锉)和B组(使用手持不锈钢K锉)。A组患者32例,患牙34颗;B组患者28例,患牙31颗。比较两组患者根管预备时间、根管填充时间、根管预备形态、根管充填质量、术后疼痛程度及不良事件发生情况。结果:A组根管预备时间、根管填充时间短于B组(P<0.05),根管预备形态良好率、根管恰填率高于B组(P<0.05),疼痛反应率低于B组(P<0.05);两组不良事件发生率对比无明显差异(P>0.05)。结论:在根尖周病变患者的根管治疗中,Protaper机械镍钛锉的应用效果更明显,能有效缩短根管预备和填充时间,改善根管预备形态,提高根管充填质量,降低患者疼痛程度。Objective:To compare the application effect of Protaper mechanical nickel-titanium file and hand-held stainless steel K-type file in root canal therapy for patients with periapical disease.Methods:In the retrospective study,60 patients with periapical disease(65 affected teeth)admitted to the hospital were divided into group A(Protaper mechanical nickel-titanium file,32 cases,34 affected teeth)and group B(hand-held stainless steel K-type file,28 cases,31 affected teeth)according to different instruments in root canal preparation between January 2023 and September 2024.The root canal preparation time,root canal filling time,root canal preparation form,root canal filling quality,postoperative pain degree and occurrence of adverse events were compared between the two groups.Results:The preparation time and filling time of root canals in group A were shorter than those in group B(P<0.05),good rate of root canal preparation form and appropriate rate of root canal filling were higher than those in group B(P<0.05),and pain response rate was lower than that in group B(P<0.05).There was no significant difference in the incidence of adverse events between the two groups.Conclusion:The application effect of Protaper mechanical nickel-titanium file is more significant in root canal therapy for patients with periapical disease,which can effectively shorten preparation time and filling time of root canals,improve root canal preparation form,root canal filling quality and relieve pain.
