健脾补肾化浊解毒方对5/6肾切除慢性肾衰竭大鼠NF-κB p65、TNF-α、IL-6、Col-Ⅳ表达的影响

摘  要:目的探讨基于“浊毒理论”指导下的健脾补肾化浊解毒方对5/6肾切除慢性肾衰竭大鼠肾组织中核转录因子κB p65(NF-κB p65)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)及Ⅳ型胶原(Col-Ⅳ)表达的影响。方法取60只雄性SD大鼠,随机选择其中12只作为假手术组,其余大鼠实施5/6肾切除手术建立慢性肾衰竭模型。将成模存活大鼠随机分为模型组12只、氯沙坦组11只、尿毒清组11只、健脾补肾化浊解毒方组11只,氯沙坦组给予氯沙坦钾5.25 mg/(kg·d)灌胃,尿毒清组给予尿毒清颗粒2.10 g/(kg·d)灌胃,健脾补肾化浊解毒方组给予健脾补肾化浊解毒方2.35 g/(kg·d)灌胃,假手术组和模型组给予等体积0.9%NaCl灌胃,均1次/d。连续灌胃12周后,检测各组大鼠血清肌酐(SCr)、血尿素氮(BUN)水平,HE染色观察残余肾组织病理形态,Western blot法与RT-qPCR法分别检测残余肾组织中NF-κB p65、TNF-α、IL-6、Col-Ⅳ蛋白和mRNA表达情况。结果模型组大鼠血清SCr、BUN水平和肾组织中NF-κB p65、TNF-α、IL-6、Col-Ⅳ蛋白及mRNA相对表达量均明显高于假手术组(P均<0.05);各药物组大鼠血清SCr、BUN水平和肾组织中NF-κB p65、TNF-α、IL-6、Col-Ⅳ蛋白及mRNA相对表达量均明显低于模型组(P均<0.05),且健脾补肾化浊解毒方组大鼠血清SCr、BUN水平和肾组织中Col-Ⅳ蛋白及mRNA相对表达量均明显低于氯沙坦组和尿毒清组(P均<0.05)。结论健脾补肾化浊解毒方可明显改善慢性肾衰竭大鼠的肾功能及肾组织病理改变,其作用机制可能与降低NF-κB p65、TNF-α、IL-6、Col-Ⅳ的表达,从而改善微炎症状态,减少细胞外基质沉积,减轻肾间质纤维化有关。Objective It is to investigate the effects of decoction for fortifying spleen,tonifying kidney and resolving turbidity(DFSTKRT)under the guidance of“turbid toxin theory”on the expressions of nuclear factorκB p65(NF-κB p65),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and typeⅣcollagen(Col-Ⅳ)in renal tissue of rats with chronic renal failure after 5/6 nephrectomy.Methods Sixty male SD rats were taken,12 of which were randomly selected as sham operation group,and the remaining rats were subjected to 5/6 nephrectomy to establish models of chronic renal failure.The surviving modeled rats were randomly divided into model group(n=12),losartan potassium group(n=11),Niaoduqing group(n=11),DFSTKRT group(n=11),the chlorosartan group was given losartan potassium 5.25 mg/(kg·d)by gavage,the Niaoduqing group was given Niaoduqing granules 2.10 g/(kg·d)by gavage,the DFSTKRT group was given DFSTKRT 2.35 g/(kg·d)by gavage,and the sham operation group and model group were given equal volume of 0.9%NaCl by gavage,all once daily.After 12 weeks of continuous gavage,the levels of serum creatinine(SCr)and blood urea nitrogen(BUN)in rats of each group were detected,the pathological morphology of residual kidney tissues was observed by HE staining,and the protein and mRNA expressions of NF-κB p65,TNF-α,IL-6,and Col-Ⅳin residual kidney tissues were respectively detected by Western blot and RT-qPCR methods.Results The levels of serum SCr,BUN and relative protein and mRNA expressions of NF-κB p65,TNF-α,IL-6,Col-Ⅳin renal tissues of rats in the model group were significantly higher than those in the sham operation group(all P<0.05);the levels of serum SCr,BUN and relative protein and mRNA expressions of NF-κB p65,TNF-α,IL-6,Col-Ⅳin renal tissues of rats in the treatment groups were significantly lower than those in the model group(all P<0.05),and the levels of serum SCr,BUN and relative protein and mRNA expressions of Col-Ⅳin renal tissues of rats in the treatment groups were significantly lower than those i
