摘 要:目的探讨超声引导下穿刺活检(US-CNB)联合核磁共振成像(MRI)对乳腺癌的诊断价值。方法选取2022年1月至2023年12月于安阳市肿瘤医院就诊的98例疑似乳腺癌患者,均接受US-CNB、MRI检查,以手术或病理活检结果作为诊断“金标准”,比较US-CNB、MRI单独及联合诊断乳腺癌的效能情况,并对其与金标准的一致性进行分析。结果98例疑似乳腺癌患者经手术或病理活检结果显示,53例恶性(乳腺癌),45例良性;US-CNB检查结果显示,49例恶性,49例良性;MRI检查结果显示,47例恶性,51例良性;联合诊断结果显示,60例恶性,38例良性。US-CNB、MRI单独诊断乳腺癌的灵敏度分别为75.47%、73.58%,均低于二者联合诊断(96.23%),漏诊率分别为24.53%、26.42%,均高于联合诊断,差异存在统计学意义(P<0.05);US-CNB、MRI单独诊断与手术或病理活检结果的一致性Kappa值分别为0.729、0.586,符合率分别为0.846、0.769;US-CNB、MRI二者联合诊断与手术或病理活检结果的一致性Kappa值为0.768,符合率为0.872,高于单独诊断(P<0.05)。结论US-CNB联合MRI可显著提高乳腺癌的诊断灵敏度,降低乳腺癌的漏诊率,在乳腺癌诊断中具有较高的应用价值。Objective To explore the diagnostic value of ultrasound-guided core needle biopsy(US-CNB)combined with magnetic resonance imaging(MRI)for breast cancer.Methods 98 patients with suspected breast cancer admitted to Anyang Cancer Hospital from January 2022 to December 2023 were selected and underwent US-CNB,MRI examinations.The results of surgery or pathological biopsy were used as the"gold standard"for diagnosis.The efficacy of US-CNB,MRI alone and in combination in the diagnosis of breast cancer was compared,and the consistency with the gold standard was analyzed.Results 98 cases of suspected breast cancer patients by surgery or pathological biopsy results showed that 53 cases were malignant(breast cancer),45 cases were benign;the results of US-CNB examination showed that 49 cases were malignant and 49 cases were benign;MRI examination results showed that 47 cases were malignant and 51 cases were benign;the results of combined diagnosis showed that 60 cases were malignant and 38 cases were benign.The sensitivities of US-CNB and MRI in the diagnosis of breast cancer were 75.47%and 73.58%,respectively,lower than that of the combined diagnosis(96.23%).The missed diagnosis rates were 24.53%and 26.42%,respectively,higher than that of the combined diagnosis,with statistically significant difference(P<0.05).The Kappa values of consistency between US-CNB,MRI and surgical or pathological biopsy results were 0.729 and 0.586,respectively,and the coincidence rates were 0.846 and 0.769,respectively.The Kappa value of consistency between the combined diagnosis of US-CNB,MRI and surgical or pathological biopsy results was 0.768,and the coincidence rate was 0.872,higher than that of single diagnosis(P<0.05).Conclusions US-CNB combined with MRI can significantly improve the diagnostic sensitivity of breast cancer,reduce the rate of missed diagnosis of breast cancer,and has higher application value in the diagnosis of breast cancer.
