针灸治疗缺血性脑卒中后运动性失语的效果

摘　　要：目的:探究针灸治疗缺血性脑卒中后运动性失语的效果。方法:选取2022年1—12月鄄城县中医医院康复科收治的76例缺血性脑卒中后运动性失语患者为研究对象。按照简单随机分组法,分成对照组和观察组,各38例。前者、后者分别接受康复治疗、康复治疗联合针灸治疗。比较两组治疗效果、言语功能、神经功能、生活质量及神经营养因子。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05);观察组汉语失语检查表(ABC)评分高于对照组,差异有统计学意义(P<0.05);观察组美国国立卫生研究院卒中量表(NHISS)评分低于对照组,生活质量综合评定问卷(GQOLI-74)评分高于对照组,差异有统计学意义(P<0.05);观察组脑源性神经营养因子(BDBF)、碱性成纤维细胞生长因子(bFGF)、胰岛素样生长因子-1(IGF-1)、神经生长因子(NGF)水平高于对照组,差异有统计学意义(P<0.05)。结论:针灸治疗缺血性脑卒中后运动性失语患者疗效优良,可通过调节神经营养因子水平,改善言语功能、神经功能,提高生活质量,利于患者早日康复。Objective:To explore the effect of acupuncture in the treatment of motor aphasia after ischemic stroke.Method:A total of 76 patients with motor aphasia after ischemic stroke admitted to the Rehabilitation Department of Juancheng County Hospital of Traditional Chinese Medicine from January to December 2022 as the research and analysis objects.According to the simple random grouping method,the patients were divided into control group and observation group,with 38 cases in each group.The former and the latter received rehabilitation treatment,rehabilitation treatment and acupuncture treatment respectively.The therapeutic effects,speech function,neurological function,quality of life,and neurotrophic factors between two groups were compared.Result:The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).The score of Chinese aphasia checklist(ABC)in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The score of National Institutes of Health stroke scale(NHISS)in the observation group was lower than that in the control group,and the score of comprehensive quality of life assessment questionnaire(GQOLI-74)was higher than that in the control group,the differences were statistically significant(P<0.05).The levels of brainderived neurotrophic factor(BDNF),basic fibroblast growth factor(bFGF),insulin-like growth factor-1(IGF-1)and nerve growth factor(NGF)in observation group were higher than those in control group,and the differences were statistically significant(P<0.05).Conclusion:Acupuncture has a good effect on patients with motor aphasia after ischemic stroke.It can improve speech function,nerve function and quality of life by adjusting the level of neurotrophic factors,which is beneficial to the early recovery of patients.
