摘　　要：目的观察中药敷脐联合膳食干预治疗慢性肾功能衰竭合并便秘患者的效果及对肾功能、生活质量的影响。方法选取2021年1月至2023年1月淮北市人民医院肾脏内科收治的80例慢性肾功能衰竭合并便秘患者,按照随机数字表法分为观察组、对照组,各40例。对照组进行膳食干预,观察组以中药敷脐联合膳食干预。比较2组便秘症状、炎症因子、生活质量、肾功能变化。结果干预后,2组便秘患者症状自评量表(PAC-SYM)粪便性状、直肠症状、腹部症状得分和总分,以及炎症因子白细胞介素6(IL-6)、C反应蛋白(CRP)水平均低于干预前(P<0.05),且观察组均低于对照组(P<0.05)。干预后,2组生活质量综合评定问卷(GQOLI-74)躯体、心理、社会和物质生活功能评分均高于干预前(P<0.05),且观察组均高于对照组(P<0.05)。干预后,2组肾功能指标β_(2)-微球蛋白(β_(2)-MG)、尿素氮(BUN)、血肌酐(SCr)水平均低于干预前(P<0.05),且观察组均低于对照组(P<0.05)。结论中药敷脐联合膳食干预能有效缓解慢性肾功能衰竭合并便秘患者的便秘症状,抑制炎症因子,改善肾功能,提高患者生活质量。Objective To investigate the effect of Chinese medicine umbilical compress combined with dietary intervention on renal function index and quality of life in chronic renal failure(CRF)patients with constipation.Methods Eighty CRF patients with constipation admitted to the Department of Nephrology,Huaibei People's Hospital from January 2021 to January 2023 were randomized 1∶1 to receive dietary intervention(control group)or umbilical compress+dietary intervention(observation group).The constipation symptoms,inflammatory factors,quality of life and renal function index were included as comparisons between groups.Results After intervention,the both groups had significantly lower dimension points(fecal traits,rectal symptoms,and abdominal symptoms)and total scores of Patient Assessment of Constipation symptom(PAC-SYM),receded inflammatory factors(interleukin-6[IL-6]and C-reactive protein[CRP]),upgraded general quality of life inventory-74(GQOLI-74)score(physical,psychological,social and material life function),decreased renal function indexes(β_(2)-microglobulin[β_(2)-MG],blood urea nitrogen[BUN)and serum creatinine[Scr])than before intervention(all P<0.05),the observation group was significantly better than the control group for those indexes(all P<0.05).Conclusion The combination of the two was effective in the treatment of CRF with constipation,which can effectively relieve constipation symptoms,inhibit inflammatory factors,improve renal function,and enhance the quality of life.
