摘  要:目的探讨薄层液基细胞学检查(TCT)、人乳头瘤病毒(HPV)基因分型及DNA定量检测对子宫颈癌前病变的诊断价值。方法选取2020年2月至2022年8月湖南旺旺医院妇产科经门诊筛查的86例疑似子宫颈癌前病变患者为研究对象。所有患者均进行TCT、HPV基因分型及DNA定量检测。以病理学检查结果作为金标准,比较三种方法单项及联合检查的诊断效能。结果86例患者中,病理学检查结果显示,阳性68例,阴性18例;TCT检查结果显示,阳性39例,阴性47例;HPV基因分型检查结果显示,阳性49例,阴性37例;DNA定量检测结果显示,阳性52例,阴性34例;三种方法联合检查结果显示,阳性65例,阴性21例。Kappa一致性检验结果显示,联合检查的一致性高于TCT、HPV基因分型、DNA定量单项检查(P<0.05)。联合检查对子宫颈癌前病变的诊断灵敏度、准确度、阴性预测值高于TCT、HPV基因分型、DNA定量单项检查(P<0.05);联合检查与TCT、HPV基因分型、DNA定量单项检查的特异度无显著差异(P>0.05);联合检查的阳性预测值高于TCT检查(P<0.05);联合检查与HPV基因分型、DNA定量单项检查的阳性预测值无显著差异(P>0.05)。结论TCT、HPV基因分型、DNA定量联合检查对子宫颈癌前病变具有较高的诊断价值,能够及时、准确地识别疾病,为临床治疗决策提供参考,值得在临床实践中推广应用。Objective To investigate the diagnostic value of thinprep cytology test(TCT),human papilloma virus(HPV)genotyping and DNA quantitative detection in cervical precancerous lesions.Methods From February 2020 to August 2022,86 patients with suspected cervical precancerous lesions screened by the outpatient in the gynecology and obstetrics department of Hunan Want Want Hospital were selected as the research objects.All patients underwent TCT,HPV genotyping and DNA quantitative detection.The pathological detection results were used as the gold standard to compare the diagnostic efficacy of single and combined detection of the three methods.Results Among the 86 patients,the results of pathological detection showed that 68 cases were positive and 18 cases were negative;TCT detection results showed that 39 cases were positive and 47 cases were negative;the results of HPV genotyping detection showed that 49 cases were positive and 37 cases were negative;the results of DNA quantitative detection showed that 52 cases were positive and 34 cases were negative;the combined detection results of the three methods showed that 65 cases were positive and 21 cases were negative.Kappa consistency test results showed that the consistency of combined detection was higher than that of TCT,HPV genotyping and DNA quantitative single detection(P<0.05).The diagnostic sensitivity,accuracy and negative predictive value of combined detection for cervical precancerous lesions were higher than those of TCT,HPV genotyping and DNA quantitative single detection(P<0.05);there was no significant difference in specificity between combined detection and TCT,HPV genotyping and DNA quantitative single detection(P>0.05);the positive predictive value of combined detection was higher than that of TCT detection(P<0.05);there was no significant difference in positive predictive value between combined detectinon and HPV genotyping and DNA quantitative single detection(P>0.05).Conclusion The combined detection of TCT,HPV genotyping and DNA quantification has hig
