外周血NLR、PLR、SII对进展期NSCLC患者化疗疗效的预测价值

摘　　要：目的 探究外周血中性粒细胞淋巴细胞比值(NLR)、血小板淋巴细胞比值(PLR)、系统免疫炎症指数(SII)对进展期非小细胞肺癌(NSCLC)患者化疗疗效的评估价值。方法 收集103例进展期NSCLC患者,按疗效分为良好组(n=79例)和不良组(n=24例)。分析外周血NLR、PLR、SII对进展期NSCLC患者化疗疗效的预测价值。结果 两组一般资料(年龄、性别、病理类型、分化程度、吸烟、饮酒)比较差异不显著(P>0.05)。不良组NLR、PLR、SII均高于良好组,差异显著(t=3.924,P<0.001、t=7.648,P<0.001、t=3.266,P=0.002)。NLR、PLR、SII及三者联合对进展期NSCLC患者化疗疗效的预测效能良好,三者联合的受试者操作特征曲线下面积(AUC)最大,为0.957,灵敏度、特异度分别为0.778、0.861。NLR的预测阈值为3.290,PLR的预测阈值为163.985,SII的预测阈值为1015.615。结论 NLR、PLR、SII能够作为进展期NSCLC患者化疗疗效的有效评估因子,三者联合应用对患者化疗疗效的预测效能最高。Objective To investigate the usefulness of the systemic immunoinflammatory index(SII),platelet lymphocyte ratio(PLR),and peripheral blood neutrophil lymphocyte ratio(NLR)in evaluating the effectiveness of treatment in patients with advanced non-small cell lung cancer(NSCLC).Methods 103 patients with advanced NSCLC,according to the efficacy of chemotherapy,were divided into good group(n=79 cases)and bad group(n=24 cases).To analyze the predictive value of peripheral blood NLR,PLR and SII for chemotherapy efficacy in patients with advanced NSCLC.Results Age,sex,pathological type,degree of differentiation,drinking,and smoking did not significantly differ between the 2 groups(P>0.05).In comparison to the good group,the NLR,PLR,and SII in the poor group were considerably higher(t=3.924,P<0.001,t=7.648,P<0.001,t=3.266,P=0.002).In patients with advanced non-small cell lung cancer(NSCLC),NLR,PLR,SII,and combination treatment exhibit strong predictive performance;the combined participants'Area Under the Curve(AUC)was 0.957,with sensitivity and specifity values of 0.778 and 0.861.For NLR,it was 3.290;for PLR,it was 163.985;and for SII,it was 1015.615.Conclusion NLR,PLR and SII can be used as effective evaluation factors for chemotherapy efficacy in patients with advanced NSCLC,and the combination of the 3 can predict the chemotherapy efficacy of patients with advanced NSCLC.
