摘 要:目的 分析白细胞介素-1β(IL-1β)、干扰素-γ(IFN-γ)、诱导型一氧化氮合酶(iNOS)表达水平与膀胱癌术后感染的相关性。方法 收集96例膀胱癌患者临床资料。依照患者术后感染情况分为非感染组(76例)和感染组(20例)。比较两组年龄、体质指数、性别、文化程度、肿瘤分期、吸烟、饮酒、IL-1β、IFN-γ、iNOS表达水平;分析IL-1β、IFN-γ、iNOS表达水平与膀胱癌术后感染的相关性;受试者操作特征曲线(ROC)下面积(AUC)评估IL-1β、IFN-γ、iNOS表达水平对膀胱癌术后感染的预测效能。结果 两组年龄、体质指数、性别、文化程度、肿瘤分期、吸烟、饮酒比较差异无统计学意义(P>0.05)。感染组IL-1β、IFN-γ、iNOS水平均高于非感染组,差异有统计学意义(t=14.278,P=0.000、t=7.633,P=0.000、t=11.749,P=0.000)。IL-1β、IFN-γ、iNOS水平与膀胱癌术后感染呈显著正相关性,P均<0.05。将膀胱癌术后感染=1,非感染=0作为因变量,IL-1β、IFN-γ、iNOS水平作为自变量,进行二元逻辑回归分析,并进行受试者操作特征曲线(ROC)分析,结果显示,IL-1β、IFN-γ、iNOS三者联合预测膀胱癌术后感染的预测效能相比单一指标较高,三者联合的ROC下面积(AUC)最大,为0.967,灵敏度为0.850,特异度为0.908。IL-1β、IFN-γ、iNOS的预测阈值分别为1.785 pg/ml、40.200 pg/ml、143.430 U/ml, IL-1β、IFN-γ、iNOS的AUC分别为0.938、0.889、0.908。结论 IL-1β、IFN-γ、iNOS在膀胱癌术后感染患者中表达水平显著升高,IL-1β、IFN-γ、iNOS与膀胱癌术后感染呈正相关关系,三者联合对于膀胱癌术后感染有良好的预测价值,可作为临床上膀胱癌术后感染的评估因子。Objective The expression levels of interleukin-1β(IL-1β),interferon-γ(IFN-γ),inducible nitric oxide synthase(iNOS)and postoperative infection of bladder cancer were analyzed.Methods Clinical data of 96 patients with bladder cancer were collected.Patients were divided into non-infected group(76 cases)and infected group(20 cases)according to postoperative infection.Age,body mass index,sex,education,tumor stage,smoking,drinking,IL-1β,IFN-γ,iNOS expression levels were compared between the 2 groups.The expression levels of IL-1β,IFN-γand iNOS were correlated with postoperative infection of bladder cancer.The area under receiver Operating characteristic curve(ROC)(AUC)was used to evaluate the predictive efficacy of IL-1β,IFN-γand iNOS expression levels in postoperative infection of bladder cancer.Results There was no significant difference in age,body mass index,sex,education level,tumor stage,smoking and drinking between the 2 groups(P>0.05).The levels of IL-1β,IFN-γand iNOS in the infected group were higher than those in the non-infected group,with statistical significance(t=14.278,P=0.000,t=7.633,P=0.000,t=11.749,P=0.000).The levels of IL-1β,IFN-γand iNOS were significantly positively correlated with postoperative infection of bladder cancer,all P<0.05.With postoperative infectio n=1,non-infectio n=0 as dependent variables and IL-1β,IFN-γand iNOS levels as independent variables,binary logistic regression analysis and receiver operating characteristic curve(ROC)analysis were performed.The combined IL-1β,IFN-γand iNOS predicted postoperative infection of bladder cancer with higher efficacy than that of single index.The combined IL-1β,IFN-γand INOS had the largest area under ROC AUC(0.967),sensitivity(0.850)and specificity(0.908).The predicted thresholds of IL-1β,IFN-γand iNOS were 1.785 pg/ml,40.200 pg/ml and 143.430 U/ml,respectively,and the AUC of IL-1β,IFN-γand iNOS were 0.938,0.889 and 0.908,respectively.Conclusion The expression levels of IL-1β,IFN-γand iNOS are significantly increased i
