摘  要:目的 探究HBP、PCT、MMP-9对脑肿瘤患者术后颅内感染的评估价值。方法 回顾性收集80例脑肿瘤患者临床资料进行研究,根据其术后颅内感染情况将患者分为感染组和未感染组。比较2组患者的一般资料、HBP、PCT、MMP-9水平,分析HBP、PCT、MMP-9表达水平对脑肿瘤患者术后并发颅内感染的预测价值。结果 2组年龄、性别、肿瘤类型、糖皮质激素使用史、预防性使用抗菌药比较,差异无统计学意义(P>0.05)。感染组HBP为(12.23±3.11)μg/L、PCT为(0.86±0.25)μg/L、MMP-9为(122.25±19.25)μg/L,均高于未感染组的(5.39±2.02)μg/L、(0.32±0.08)μg/L、(25.37±5.78)μg/L,差异有统计学意义(P均<0.001)。脑肿瘤患者脑脊液HBP、PCT、MMP-9对术后并发颅内感染的预测价值较高,AUC分别为0.885、0.907、0.934,三者联合AUC最大(0.975),灵敏度(0.901)及特异度(0.958)均较高,预测效能最佳。HBP的预测阈值为8.430μg/L、PCT的预测阈值为0.600μg/L、MMP-9的预测阈值为46.250μg/L。结论 脑肿瘤术后并发颅内感染患者脑脊液HBP、PCT、MMP-9表达水平异常升高,联合检测HBP、 PCT和MMP-9能够更好地预测脑肿瘤患者术后颅内感染的发生,可将其应用于脑肿瘤术后并发颅内感染的评估。Objective To explore the evaluation value of HBP,PCT and MMP-9 in intracranial infection of patients with brain tumor after surgery.Methods The clinical data of 80 patients with brain tumor were retrospectively collected and divided into infected group and uninfected group according to their postoperative intracranial infection.The general data,HBP,PCT and MMP-9 levels of the 2 groups were compared,and the value of the expression levels of HBP,PCT and MMP-9 in predicting intracranial infection after surgery in patients with brain tumor was analyzed.Results There was no significant difference in age,sex,tumor type,history of glucocorticoid use and prophylactic use of antimicrobials between 2 groups(P>0.05).The levels of HBP(12.23±3.11)μg/L,PCT(0.86±0.25)μg/L and MMP-9(122.25±19.25)μg/L in infected group were higher than those in uninfected group(5.39±2.02)μg/L,(0.32±0.08)μg/L and(25.37±5.78)μg/L,the difference was statistically significant(P<0.001).Cerebrospinal fluid HBP,PCT and MMP-9 in patients with brain tumor had high predictive value for postoperative intracranial infection,with AUC of 0.885,0.907 and 0.934,respectively.The combined AUC of the three was the largest(0.975),sensitivity(0.901)and specificity(0.958),and the prediction efficacy was the best.The prediction threshold of HBP is 8.430μg/L,that of PCT is 0.600μg/L,and that of MMP-9 is 46.250μg/L.Conclusion The expression levels of HBP,PCT and MMP-9 in cerebrospinal fluid of patients with intracranial infection after brain tumor surgery are abnormally elevated.The combined detection of HBP,PCT and MMP-9 can better predict the occurrence of intracranial infection in patients with brain tumor surgery,and can be applied in the evaluation of intracranial infection after brain tumor surgery.
