摘 要:目的 分析体素内不相干运动-扩散加权成像(IVIM-DWI)技术联合纵向弛豫时间定量成像(T1 mapping)对乳腺肿瘤的定性诊断价值。方法 前瞻性研究,纳入112例乳腺肿瘤患者作为研究对象,患者符合手术治疗适应证,并同意接受手术治疗。所有患者于术前接受IVIM-DWI、T1 mapping检查,以手术组织病理活检结果将患者分为良性组和恶性组,比较2组IVIM-DWI参数[表现弥散系数(ADC)、单纯扩散系数(D)、伪扩散系数(D^(*))以及灌注分数(f)]、T1 mapping参数[T_1值],评估IVIM-DWI联合T1 mapping成像对乳腺肿瘤的定性诊断价值。结果 全部患者均完成手术组织病理活检,结果显示,112例乳腺肿瘤患者经活检确诊恶性12例,占比10.71%。恶性组ADC、D值均小于良性组,f、D^(*)、T_(1)值均大于良性组,差异有统计学意义(P<0.05)。绘制ROC曲线,IVIM-DWI、T1 mapping主要参数(ADC、D、f、D^(*)、T_(1)值)对乳腺肿瘤定性诊断的ACU均>0.70,有一定诊断价值。基于上述参数构建列线图模型发现,ADC、D越小,f、D^(*)、T_(1)值越大的乳腺肿瘤患者,恶性风险越高,列线图中各参数联合对乳腺肿瘤有较高的定性诊断价值(AUC=0.980)。结论 IVIM-DWI技术联合T1 mapping成像可用于对乳腺肿瘤的定性诊断,基于各参数构建的列线图模型可对乳腺肿瘤作出定性诊断。Objective To analyze the qualitative diagnostic value of intravoxel incoherent morion-diffusion weighted imaging(IVIM-DWI)combined with longitudinal relaxation time quantitative imaging(T1 mapping)for breast tumors.Methods A prospective study was conducted,including 112 patients with breast tumors as the study subjects.The patients met the indications for surgical treatment and agreed to undergo surgical treatment.All patients underwent IVIM-DWI and T1 mapping before surgery.The patients were divided into benign group and malignant group according to the pathological biopsy results of surgical tissues.The IVIM-DWI parameters[apparent diffusion coefficient(ADC),pure diffusion coefficient(D),pseudo-diffusion coefficient(D*)and perfusion fraction(f)]and T1 mapping parameters[T 1 value]were compared between the 2 groups to evaluate the qualitative diagnostic value of IVIM-DWI combined with T1 mapping imaging for breast masses.Results All patients completed surgical tissue pathological biopsy,and the results showed that 12 out of 112 breast tumor patients were diagnosed with malignancy through biopsy,accounting for 10.71%.The ADC and D values of the malignant group were lower than those of the benign group,while the f,D*,and T 1 values were higher than those of the benign group,with statistical significant differences(P<0.05).The ROC curve was drawn.The main parameters of IVIM-DWI and T1 mapping(ADC,D,f,D*,T 1 value)were all>0.70 in the ACU of qualitative diagnosis of,which had certain diagnostic value.Based on the above parameters,the nomogram model was constructed.It was found that the smaller the ADC and D,the greater the f,D*and T 1 values,the higher the risk of malignancy in patients with breast masses.The combination of parameters in the nomogram had a high qualitative diagnostic value for breast masses(AUC=0.980).Conclusion The combination of IVIM-DWI technology and T1 mapping imaging can be used for qualitative diagnosis of breast tumors.A column chart model constructed based on various parameters can make qua
