低温等离子联合鼻内镜在复发性鼻腔鼻窦恶性肿瘤治疗中的应用价值

摘　　要：目的 探究低温等离子联合鼻内镜在复发性鼻腔鼻窦恶性肿瘤治疗中的应用价值。方法 筛选92例复发性鼻腔鼻窦恶性肿瘤患者纳入研究,根据其实际治疗方法,将患者分为鼻内镜治疗的对照组(n=59)、低温等离子联合鼻内镜治疗的观察组(n=33)。比较2组一般资料、手术情况、并发症发生率、临床效率。结果 2组患者手术情况无差异(P=0.924)。对照组手术时间更短,观察组术中出血量更少且住院时间更短,其差异均具有统计学意义(P均<0.001)。2组均发生视力下降、流泪和神经麻木的并发症,对照组还观察到4例复视,但2组并发症分布及其总发生率无明显差异(P>0.05)。2组患者治疗有效率无明显差异(P=0.099),但观察组治愈和好转分布明显优于对照组(P=0.023)。术前和术后12个月2组患者VAS评分无明显差异(P>0.05),但术后3个月和术后6个月观察组患者VAS评分明显更低(P<0.001)。2组患者肿瘤复发情况无明显差异(P>0.05)。结论 低温等离子联合鼻内镜适宜治疗复发性鼻腔鼻窦恶性肿瘤,具有出血量少、术后住院时间短、疼痛感低、患者恢复较快、可保护正常解剖结构的优点。Objective To investigate the application value of low-temperature plasma combined with nasal endoscopy in the treatment of recurrent nasal sinus malignant tumors.Methods Screening 92 patients with recurrent malignant tumors of the nasal cavity and sinuses for inclusion in the study.According to their actual treatment methods,the patients were divided into the control group with nasal endoscopic treatment(n=59)and the observation group with low-temperature plasma combined with nasal endoscopic treatment(n=33).The general information,surgical situation,complication rate,and clinical efficiency of the 2 groups were compared.Results There was no difference in the surgical situation between the 2 groups(P=0.924).The operation time was shorter in the control group,and intraoperative bleeding was smaller and hospitalization time was shorter in the observation group,and the differences were statistically significant(all P<0.001).Complications of decreased visual acuity,tearing and nerve numbness occurred in both groups,and 4 cases of diplopia were also observed in the control group,but there was no significant difference in the distribution of complications and their overall incidence between the 2 groups(P>0.05).There was no significant difference in treatment efficiency between the 2 groups(P=0.099),but the distribution of cure and improvement was significantly better in the observation group than in the control group(P=0.023).There was no significant difference in the VAS scores between the two groups of patients in the preoperative and postoperative 12 months(P>0.05),and the VAS of patients in the observation group was significantly lower in the postoperative 3 months and postoperative 6 months(P<0.001).There was no significant difference in tumor recurrence between the 2 groups(P>0.05).Conclusion Low-temperature plasma combined with nasal endoscopy is suitable for the treatment of recurrent nasal sinus malignant tumors,which has the advantages of less bleeding,shorter postoperative hospital stay/less pain,faster rec
