夹线牵引辅助ESD术治疗食管早癌及癌前病变的临床疗效及安全性

摘　　要：目的 探讨夹线牵引辅助内镜黏膜下剥离术(ESD)治疗食管早癌及癌前病变的临床疗效及安全性。方法选择食管早癌及癌前病变患者78例,通过随机数字表法分为2组,各39例。对照组行ESD治疗,观察组行夹线牵引辅助ESD术。比较2组病灶切除效果、手术时长、并发症发生情况以及住院时间。结果 2组病灶切除效果相当,但观察组手术时间[(47.73±5.99)min]、住院时间[(7.81±1.92)d]短于对照组[(70.21±6.83)min、(11.35±2.18)d],并发症发生率[2.56%(1/39)]低于对照组[20.51%(8/39)],差异均有统计学意义(P<0.05)。结论 夹线牵引辅助ESD术治疗食管早癌及癌前病变,效果较好,能够彻底切除病灶,且手术操作时间短,并发症少,可缩短住院时间,减少住院成本,值得临床广泛应用。Objective To investigate the clinical efficacy and safety of wire traction assisted endoscopic submucosal dissection(ESD)in the treatment of early esophageal cancer and precancerous lesions.Methods 78 patients with early esophageal cancer and precancerous lesions were divided into 2 groups and 39 cases each by random number table.The control group underwent ESD and the observation group underwent traction-assisted ESD.The effect of lesion resection,operation duration,duration of complications and hospital stay were compared in the 2 groups.Results the effect of lesion resection in the 2 groups was comparable,but the operation time[(47.73±5.99)min]and hospitalization time[(7.81±1.92)d]were shorter than the control group[(70.21±6.83)min,(11.35±2.18)d],and the complication rate[2.56%(1/39)]was lower than the control group[20.51%(8/39)]in the control group,which was statistically significant(P<0.05).Conclusion Patients with early esophageal cancer and precancerous lesions assisted ESD have good results,can completely remove the lesion,and the operation time is short,less complications,can shorten the hospital stay and reduce the hospital cost,which is worthy of wide clinical application.
