近端胃癌患者近端胃切除双通道消化道重建不同吻合方案短期疗效的观察

摘　　要：目的 分析近端胃癌患者近端胃切除双通道消化道重建不同吻合方案的短期疗效。方法 选取86例行近端胃切除术的近端胃癌患者,所有患者均采用双通道消化道重建,其中食管空肠“π”形吻合者38例(研究组),食管空肠端侧吻合者48组(对照组)。对比两组的围术期指标,术后并发症发生率、术后生活质量。结果 研究组切口长度、手术时间均优于对照组(P<0.05),术中出血量、术后排气时间、住院时间比较差异均无统计学意义(P>0.05);两组术后并发症发生率比较差异无统计学意义(P>0.05);研究组术后生活质量评分高于对照组(P<0.05)。结论 两种吻合方案手术效果相当,但食管空肠“π”形吻合术切口长度较短,手术时间较食管空肠端侧吻合术具有优势,有利于保持患者腹部美观,改善患者术后生活质量。Objective To investigate the short-term efficacy of proximal gastrectomy with double tract reconstruction by different anastomosis techniques in patients with proximal gastric cancer.Methods 86 patients with proximal gastric cancer who underwent proximal gastrectomy were selected.All patients underwent double-channel digestive tract reconstruction,including 38 patients with esophagojejunal“π”-shaped anastomosis(study group)and 48 patients with esophagojejunal end-to-side anastomosis(control group).The perioperative indicators,postoperative complication rates,and postoperative quality of life were compared between the 2 groups.Results The incision length and operation time of the study group were better than those of the control group(P<0.05),and there was no statistically significant difference in intraoperative blood loss,postoperative exhaust time,and hospitalization time(P>0.05);postoperative complications between the 2 groups There was no statistical significance in the incidence rate(P>0.05);the postoperative quality of life score of the study group was higher than that of the control group(P<0.05).Conclusion The 2 anastomosis schemes show comparable clinical efficacy,but"π"-shaped oesophagojejunal anastomosis has the advantages of shorter the length of the incision,and shorter operation time,better abdominal aesthetics and postoperative quality of life compared to circular esophagojejunal anastomosis for patients with proximal gastric cancer underwent proximal gastrectomy and double tract reconstruction.
