超声引导腹横肌平面阻滞用于直肠癌腹腔镜术后镇痛的临床效果观察

摘　　要：目的 探讨直肠癌腹腔镜术后应用腹横肌平面(TAP)阻滞与静脉自控镇痛(PCIA)的镇痛效果。方法 选择60例直肠癌患者,均进行直肠癌腹腔镜手术治疗,术后镇痛按随机数字表法分为2组,对照组30例进行自控PCIA,观察组30例行TAP阻滞。比较2组镇痛效果、临床相关指标及不良反应发生情况。结果 术后2 h、6 h、12 h、24 h,观察组不同状态下的疼痛评分以及术后不良反应发生率均低于对照组,镇痛泵按压次数、全麻苏醒时间、肛门排气时间、尿管拔除时间、24 h内舒芬太尼用量以及住院时间均少于对照组,差异有统计学意义(P<0.05)。结论 TAP阻滞用于直肠癌腹腔镜术后镇痛的效果优于PCIA,可减轻疼痛程度,减少舒芬太尼用量与不良反应发生,促进患者术后更好更快的恢复,具有较高的推广应用价值。Objective To investigate the analgesic effect of transversus abdominal muscle plane(TAP)block and controlled intravenous analgesia(PCIA)after laparoscopic surgery for rectal cancer.Methods 60 patients with rectal cancer were selected to undergo laparoscopic surgery for rectal cancer,and were divided into 2 groups according to random number table method after surgery,30 patients in control group received automatic control PCIA,and 30 patients in the observation group received TAP block.The analgesic effect,clinical indexes and adverse reactions were compared between the 2 groups.Results At 2 h,6 h,12 h and 24h after surgery,the pain scores and the incidence of postoperative adverse reactions in the observation group were lower than those in the control group,and the number of analgesic pump compression,recovery time under general anesthesia,anal exhaust time,urinary tube removal time,sufentanil dosage within 24h and hospital stay were lower than those in the control group.The difference was statistically significant(P<0.05).Conclusion TAP block has better analgesic effect than PCIA after laparoscopic rectal cancer surgery,which can reduce the pain degree,reduce the dosage of sufentanil and the occurrence of adverse reactions,and promote better and faster postoperative recovery of patients,and has high application value.
