摘 要:目的 分析实时剪切波超声弹性成像(SWE)联合声速匹配(SSC)技术鉴别诊断肝癌与肝血管瘤的效果。方法 选取96例患者,均经病理检查明确诊断,其中41例肝癌、55例肝血管瘤,将肝癌患者划入观察组,肝血管瘤患者划入对照组;均接受实时剪切波超声弹性成像、SSC技术检查,比较两组SWE参数[杨氏模量平均值、剪切波速度最大值(V_(max))、剪切波速度平均值(V_(mean))]、SSC参数[病变区域声速匹配值(ZSI)];另绘制受试者工作曲线(ROC),分析杨氏模量平均值、V_(max)、V_(mean)、ZSI联合诊断肝癌、肝血管瘤的价值。结果 观察组的杨氏模量平均值、V_(max)、V_(mean)、ZSI高于对照组(P<0.05);ROC结果显示:杨氏模量平均值联合V_(max)、V_(mean)、ZSI鉴别肝癌、肝血管瘤的曲线下面积(AUC)高于几者单独鉴别。结论 SWE联合SSC技术能够在一定程度上鉴别诊断出肝癌与肝血管瘤,具有一定的临床应用价值,值得临床推广与应用。Objective To analyze the effectiveness of real-time shear wave ultrasound elastography(SWE)combined with sound velocity matching(SSC)technology in the differential diagnosis of liver cancer and hepatic hemangioma.Methods 96 patients were selected,all of whom were diagnosed through pathological examination,including 41 cases of liver cancer and 55 cases of hepatic hemangioma.Liver cancer patients were assigned to the observation group,while hepatic hemangioma patients were assigned to the control group;Both underwent real-time shear wave ultrasound elastography and SSC technology examination,and compared the SWE parameters[mean Young's modulus,maximum shear wave velocity(Vmax),mean shear wave velocity(V_(mean))]and SSC parameters[matching value of sound velocity in the lesion area(ZSI)]between the 2 groups;Draw a receiver operating characteristic(ROC)curve to analyze the combined diagnostic value of mean Young's modulus,Vmax,Vmean,and ZSI for liver cancer and hepatic hemangioma.Results The average Young's modulus,Vmax,Vmean,and ZSI of the observation group were higher than those of the control group(P<0.05);The ROC results showed that the area under the curve(AUC)of the average Young's modulus combined with V_(max),V_(mean),and ZSI in distinguishing liver cancer and hepatic hemangioma was higher than that of distinguishing them separately.Conclusion The combination of SWE and SSC technology can differentiate and diagnose liver cancer and hepatic hemangioma to a certain extent,and has certain clinical application value,which is worthy of clinical promotion and application.
