摘　　要：目的 研究Er:YAG剥脱点阵激光联合光动力疗法对基底细胞癌患者的治疗疗效。方法 选取82例基底细胞癌患者作为研究对象,按照治疗方法的不同分为对照组和观察组两组,每组各41例,对照组采用光动力疗法联合贝复新凝胶治疗,观察组在对照组的基础上增加Er:YAG剥脱点阵激光,对比两组患者的治疗效果、美容效果、术后并发症发生率以及复发情况。结果 观察组治疗后瘢痕评分比对照组更低,差异有统计学意义(P<0.05);观察组美容优良率比对照组高,差异有统计学意义(P<0.05);观察组术后并发症发生率较对照组稍高,但两组对比没有差异(P>0.05),观察组患者复发率更低,差异有统计学意义(P<0.05)。结论 Er:YAG剥脱点阵激光与光动力疗法相结合,在基底细胞癌治疗中展现出良好的疗效和优越的美容效果,有望成为一种新型有效的微创治疗方法。Objective To investigate the significance of fractional laser combined with photodynamic therapy for Er:YAG ablative fractional therapy in the treatment of patients with basal cell carcinoma.Methods 82 subjects diagnosed with basal cell carcinoma served as the subjects of this study.These individuals were allocated into 2 distinct groups,namely the control group and the experimental group,the control group and the observation group,with 41 cases in each group,The control group was treated with photodynamic therapy combined with Beifuxin gel,and the observation group was treated with Er:YAG ablative fractional laser on the basis of the control group.the treatment effect,cosmetic effect,incidence of postoperative complications and recurrence during long-term follow-up were compared between the 2 groups.Results The scar score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05),the cosmetic improvement rate of the patients in the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05),the incidence of postoperative complications in the observation group was slightly higher than that in the control group,but there was no difference between the 2 groups(P>0.05),and the recurrence rate of the patients in the observation group was lower and the difference was statistically significant(P<0.05).Conclusion The combination of Er:YAG ablative fractional laser and photodynamic therapy has shown good efficacy and superior cosmetic effect in the treatment of basal cell carcinoma,and is expected to become a new and effective minimally invasive treatment.
