Er:YAG激光在侵袭性牙周炎患者牙周正畸联合治疗中的应用及对口腔菌群的影响

摘　　要：目的探讨Er:YAG激光在侵袭性牙周炎(AgP)患者牙周正畸联合治疗中应用的效果及对口腔菌群的影响。方法选取濮阳市第三人民医院2019年1月至2023年6月收治的86例接受牙周正畸联合治疗的AgP患者,采用SPSS统计软件生成随机数字表,并按照随机分组原则将患者分为研究组和对照组,各43例。研究组和对照组患者均接受牙周正畸联合基础治疗措施,研究组患者同时接受Er:YAG激光治疗。对比两组患者治疗前后的牙周健康指标、龈沟液中炎症因子、口腔菌群水平以及治疗效果的差异。结果治疗后,研究组患者的牙周探诊深度(PD)、临床附着丧失(CAL)、菌斑指数(PLI)均低于对照组(P<0.05);治疗后,研究组患者的龈沟液胸腺基质淋巴细胞生成素(TSLP)、白细胞介素-33(IL-33)、单核细胞趋化因子(MCP-1)均低于对照组(P<0.05);治疗前,研究组和对照组患者口腔内伴放线聚集杆菌、牙龈卟啉单胞菌、具核梭杆菌、乳杆菌属、双歧杆菌属、酵母菌属检测结果比较,差异无统计学意义(P>0.05);治疗后,研究组患者的口腔内伴放线聚集杆菌、牙龈卟啉单胞菌水平低于对照组,乳杆菌属、双歧杆菌属检出值高于对照组(P<0.05);治疗后对两组患者进行疗效评价,研究组患者显效率为74.42%、有效率为25.58%,对照组显效率为48.84%、有效率为46.51%、无效率为4.65%,研究组治疗效果优于对照组(P<0.05)。结论AgP患者牙周正畸联合治疗中应用Er:YAG激光辅助治疗,有助于进一步改善牙周健康指标,减轻炎症反应程度,改善口腔菌群并提升治疗效果。Objective To investigate the effect of Er:YAG laser in combined periodontal orthodontic treatment of patients with aggressive periodontitis(AgP)and its influence on oral flora.Methods A total of 86 AgP patients who received periodontal orthodontic combined treatment in Third People’s Hospital of Puyang from January 2019 to June 2023 were selected.SPSS statistical software was used to generate a random number table,and the patients were divided into study group and control group according to the principle of random grouping,with 43 cases in each group.Both the study group and the control group patients received periodontal orthodontic combined basic treatment measures.The patients in the study group also received Er:YAG laser treatment.The differences in periodontal health indicators,inflammatory cytokines in gingival crevicular fluid,oral microflora levels,and treatment effects before and after treatment between the two groups were compared.Results After treatment,the probing depth(PD),clinical attachment loss(CAL),and plaque index(PLI)levels in the study group were lower than those in the control group(P<0.05).Additionally,the levels of thymic stromal lymphopoietin(TSLP),interleukin-3(IL-33),and monocyte chemoattractant protein-1(MCP-1)in gingival crevicular fluid were lower in the study group compared to the control group(P<0.05).Before treatment,there were no significant differences between the study and control groups in the presence of Aggregatibacter actinomycetemcomitans,Porphyromonas gingivalis,Fusobacterium nucleatum,Lactobacillus,Bifidobacterium,and Saccharomycete(P>0.05).After treatment,the levels of Aggregatibacter actinomycetemcomitans and Porphyromonas gingivalis were lower in the study group than in the control group,while the levels of Lactobacillus and Bifidobacterium were higher(P<0.05).The efficacy evaluation post-treatment showed that the study group had a markedly effective rate of 74.42%and an effective rate of 25.58%,compared to the control group’s markedly effective rate of 48.84%,effe
