结直肠癌微卫星不稳定状态与临床病理特征分析

摘  要:目的探究结直肠癌微卫星不稳定性和错配修复蛋白的分子特征,并分析微卫星不稳定性状态与结直肠癌临床病理特征的相关性。方法收集402例结直肠癌组织病理标本,分别采用多重荧光聚合酶链式反应-毛细管电泳法和免疫组织化学染色法进行微卫星不稳定性和错配修复蛋白检测,并分析结直肠癌的分子特征及临床病理特征。结果402例结直肠癌样本中,微卫星高度不稳定型占比8.46%,微卫星低度不稳定型占比2.49%,2B3D位点中敏感性高的位点分别为BAT-26(8.46%)、BAT-25(7.96%)和D2S123(7.46%)。错配修复缺陷型发生率为6.22%,错配修复蛋白缺失形式以PMS2和MLH1共缺失为主要类型。微卫星不稳定性与错配修复蛋白的结果不符合率为3.73%。另外,微卫星不稳定性状态与结直肠癌患者的年龄、肿瘤生长部位、肿瘤直径及病理类型相关(P<0.05)。结论结直肠癌具有独特的微卫星不稳定性及错配修复系统分子特征,并且微卫星不稳定性状态与结直肠癌的临床病理特征具有相关性。Objective To investigate the molecular characteristics of microsatellite instability and mismatch repair in colorectal cancer,and analyze the correlation between microsatellite instability status and clinicopathological features of colorectal cancer.Methods A total of 402 pathological specimens of colorectal cancer were collected from the First Affiliated Hospital of Harbin Medical University.Multiple fluorescence polymerase chain reaction-capillary electrophoresis and immunohistochemical staining were used to detect microsatellite instability and mismatch repair proteins,respectively,and the molecular and clinical pathological characteristics of colorectal cancer were analyzed.Results Among the 402 colorectal cancer samples,the incidence of MSI-H and microsatellite instability-L was 8.46%and 2.49%,respectively.The most sensitive sites in the 2B3D were BAT-26(8.46%),BAT-25(7.96%),and D2S123(7.46%),respectively.The inci-dence of deficient mismatch repair was 6.22%,and the main form of deficient mismatch repair was the codele-tion of PMS2 and MLH1.The non-coincidence rate between microsatellite instability and mismatch repair was 3.73%.In addition,the microsatellite instability status was correlated with age,tumor growth site,tumor diameter and pathological type of patients with colorectal cancer,and the analysis results were statistically significant(P<0.05).Conclusion Colorectal cancer has unique microsatellite instability and mismatch repair molecular features,and microsatellite instability status is correlated with the clinical and pathological characteristics of colorectal cancer.
