摘  要:目的:探究宫颈上皮内病变2级(CIN2)患者阴道微生态特征与外周血淋巴细胞的关系。方法:回顾性选取2019年5月-2024年2月于本院就诊的CIN2患者68例为观察组,年龄匹配的健康体检非CIN女性68例为对照组,比较两组阴道微生态与外周血T淋巴细胞亚群。结果:两组洁净度、加德纳菌阳性率、真菌阳性率、pH值、过氧化氢、唾液酸苷酶、乙酰氨基葡萄糖苷酶水平无差异(P>0.05),观察组细菌性阴道病、乳酸杆菌阳性率、滴虫阳性率、白细胞酯酶、衣原体阳性率均大于对照组,血CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均低于对照组,CD8^(+)水平高于对照组(均P<0.05);乳酸杆菌与CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)呈正相关,与CD8^(+)呈负相关,而细菌性阴道病、滴虫、白细胞酯酶、衣原体均与CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)呈负相关,与CD8^(+)呈负相关(均P<0.05)。结论:CIN2级患者阴道微生态失调、T淋巴细胞亚群数量下降,纠正微生态环境、提高患者的免疫功能,对宫颈癌的防治具有积极意义。Objective:To explore the correlation between the vaginal microecological characteristics of women with cervical intraepithelial neoplasia lesion grade 2(CIN2)and their peripheral blood lymphocytes levels.Methods:68 women with CIN2 who visited the hospital from May 2019 to February 2024 were retrospectively selected in observation group,and 68 healthy women with physical examinations who had matched the age of the women in the observation group during the same period were selected in control group.The vaginal microecology and the levels of peripheral blood T lymphocyte subsets of the women were compared between the two groups.Results:There were no significant differences in the cleanliness,the positive rates of Gardnerella and fungi,the pH value,the levels of hydrogen peroxide,sialidase and acetyl glucose minidase of the women between the two groups(P>0.05).The bacterial vaginosis rate,and the positive rates of lactobacillus,trichomonas,leukocyte esterase and chlamydia of the women in the observation group were significantly higher than those of the women in the control group.The levels of CD3^(+)and CD4^(+),and the CD4^(+)/CD8^(+)value of the women in the observation group were significantly lower than those of the women in the control group,and the level of CD8^(+)of the women in the observation group was significantly higher than that of the women in the control group(all P<0.05).The positive rate of lactobacillus of the women in the observation group was positively correlated with their levels of CD3^(+)and CD4^(+),and their CD4^(+)/CD8^(+)value,and which was negatively correlated with their CD8^(+)value,while the positive rates of bacterial vaginosis,trichomonas,leukocyte esterase and Chlamydia of the women in the observation group were negatively correlated with their levels of CD3^(+)and CD4^(+)and their CD4^(+)/CD8^(+)value,and which were negatively correlated with their CD8^(+)level(all P<0.05).Conclusion:The vaginal microbiosis imbalance of the women with CIN grade 2 can lead to their decreased levels of
