摘　　要：目的探讨面部旋转皮瓣修复治疗对颌面部肿瘤切除术后修复的价值及对并发症的影响。方法选取我院收治的颌面部肿瘤患者102例,均于肿瘤切除术后进行修复治疗,按信封随机法分成观察组和对照组,每组51例。对照组施行前臂皮瓣修复治疗,观察组施行面部旋转皮瓣修复治疗,对比两组围术期指标、术后满意度得分、美观度得分、瘢痕得分和并发症等。结果观察组皮瓣制作与吻合时长短于对照组(P<0.05),但两组手术时长、经口进食和住院天数比较,P>0.05。术后12个月,观察组鼻部与面侧颧颊部满意度得分高于对照组,血管分布、厚度、色泽、柔软度等瘢痕得分低于对照组(P<0.05);两组睑唇功能部与额部满意度得分比较,P>0.05。拆线后1个月、术后3个月、6个月和12个月,两组患者自主评价得分和医师评价得分均降低,且观察组得分均低于对照组(P<0.05)。观察组皮瓣感染、局部皮肤缺血和血肿形成等并发症发生率低于对照组(P<0.05)。结论在颌面部肿瘤切除术后修复治疗中,面部旋转皮瓣修复的疗效显著,有助于改善围术期指标,减少术后瘢痕,降低并发症风险,提高术后满意度和美观度。Objective To explore the value of facial rotating flap repair in maxillofacial tumor after resection and and its effect on complications.Methods 102 maxillofacial tumors patients who underwent repair treatment after tumor resection in our hospital were selected and were randomly divided into the observation group and the control group,51 cases in each group.The control group received forearm flap repair treatment,while the observation group received facial rotation flap repair treatment.The perioperative indicators,postoperative satisfaction scores,aesthetic scores,scar score and complications were compared between the two groups.Results The time of skin flap fabrication and anastomosis in the observation group was longer than those in the control group(P<0.05),but there were no significant difference in surgical time,oral feeding,and hospitalization time between the two groups(P>0.05).12 months after surgery,the satisfaction scores of the nose and facial cheekbones in the observation group were higher than those in the control group,and the scar scores such as vascular distribution,thickness,color,and softness were lower than those in the control group(P<0.05).There were no significant difference in the satisfaction scores of the eyelid lip function and forehead in the two groups(P>0.05).1 month,3 months,6 months,and 12 months after suture removal,the self-evaluation scores and physician evaluation scores in the two groups all decreased,and the observation groupwere lower than those in the control group(P<0.05).The total incidence of complications such as skin flap infection,scar hyperplasia,and hematoma formation in the observation group were lower than those in the control group(P<0.05).Conclusion In the repair treatment of maxillofacial tumors after resection,facial rotation flap repair has a significant therapeutic effect,which can improve perioperative indicators,reduce postoperative scar and complications,and promote postoperative satisfaction and aesthetic.
