数字X线摄影与CT检查联合对早期周围型肺癌的诊断效能分析

摘　　要：目的探讨数字X线摄影(digital radiography,DR)与CT在早期周围型肺癌(peripheral lung cancer,PLC)诊断中的临床价值。方法选取2020年1月—2023年12月济宁市第三人民医院收治的92例疑似PLC患者,所有患者均行DR、CT检查,以手术病理结果为金标准,比较DR、CT单独及联合检查对PLC的诊断结果及诊断效能。结果92例疑似PLC患者中,手术病理结果确诊阳性47例,阴性45例;DR检出阳性41例,阴性51例;CT检出阳性43例,阴性49例;联合检查检出阳性46例,阴性46例。联合检查的灵敏度为95.74%(45/47)、准确度为96.74%(89/92),均高于DR的78.72%(37/47)、84.78%(78/92)及CT的82.98%(39/47)、86.96%(80/92),差异有统计学意义(χ^(2)=6.060、7.938,P均<0.05)。3种检查方式的特异度、阳性预测值、阴性预测值比较,差异无统计学意义(P均>0.05)。结论DR与CT联合检查可提高PLC诊断的准确度,更利于早期治疗工作的开展。Objective To investigate the clinical value of digital radiography(DR)and CT in the diagnosis of early peripheral lung cancer(PLC).Methods A total of 92 patients with suspected PLC admitted to Jining Third People's Hospital from January 2020 to December 2023 were selected.All patients underwent DR and CT examinations,and the surgical pathological results were used as the gold standard to compare the diagnostic results and diagnostic efficacy of DR and CT alone and in combination for PLC.Results Among the 92 patients with suspected PLC,47 cases were confirmed positive and 45 cases were negative by surgical pathology.DR detection positive in 41 cases and negative in 51 cases,CT detected positive in 43 cases and negative in 49 cases,combined examination detected 46 positive cases and 46 negative cases.The sensitivity and accuracy of combined examination were 95.74%(45/47)and 96.74%(89/92),which were higher than 78.72%(37/47),84.78%(78/92)of DR and 82.98%(39/47),86.96%(80/92)of CT examination,and the differences were statistically significant(χ^(2)=6.060,7.938,both P<0.05).There were no significant differences in the specificity,positive predictive value and negative predictive value among the three methods(all P>0.05).Conclusion The combined examination of DR and CT can improve the accuracy of PLC diagnosis and is more conducive to early treatment.
