改良式TBL教学法联合Mini-CEX在呼吸内科住培教学中的应用

摘  要:目的探讨改良式基于团队的教学法(team-based learning,TBL)联合迷你临床演练评估(mini-clinical evaluation exercise,Mini-CEX)在呼吸内科住院医师规范化培训教学中的应用效果。方法选取2022年7月—2023年10月在锦州医科大学第一附属医院呼吸内科住院医师规范化培训(简称“住培”)医师72名为研究对象。采用随机数表法将住培医师分为研究组和对照组各36名,研究组采用改良式TBL教学法联合Mini-CEX,对照组采用传统教学法,教学结束后,比较2组住培医师Mini-CEX评分、考核成绩、教学效果及教学满意度。结果教学后,研究组住培医师Mini-CEX各维度评分均高于对照组,差异有统计学意义(P<0.001);教学后,研究组住培医师的理论知识考核成绩[(88.25±8.85)分]、病例分析能力[(90.16±9.18)分]、临床操作能力[(89.46±8.26)分]得分均高于对照组[(81.26±8.67)分、(82.38±8.54)分、(80.18±8.05)分],差异有统计学意义(P<0.05);教学后,研究组住培医师的教学效果各项得分均高于对照组,差异有统计学意义(P<0.001);教学后,研究组住培医师的教学满意度(94.44%)高于对照组(75.00%),差异有统计学意义(P<0.05)。结论改良式TBL教学法配合Mini-CEX可提高呼吸内科住培医师的考核成绩、教学效果及教学满意度,为后续进一步优化住培教学方法提供依据。Objective To study the application of the modified team-based learning(TBL)with mini-clinical evaluation exercise(Mini-CEX)in the standardized training of residents in respiratory medicine.Methods A total of 72 physicians who received standardized training for respiratory residents in the First Affiliated Hospital of Jinzhou Medical University from July 2022 to October 2023 were selected as the study objects.Using a random number table method,resident physicians were divided into a study group and a control group,with 36 participants in each group.A modified TBL teaching method combined with Mini-CEX was applied to the study group,while the traditional teaching method was followed by the control group.After the teaching sessions,a comparison was made between the two groups regarding Mini-CEX scores,assessment results,teaching effectiveness,and teaching satisfaction.Results After teaching,the Mini CEX scores of resident physicians in the research group were higher than those in the control group in all dimensions,and the differences were statistically significant(P<0.001);After teaching,the theoretical knowledge assessment scores of the resident training physicians in the research group[88.25±8.85 points],case analysis ability[90.16±9.18 points],and clinical operation ability[89.46±8.26 points]were all higher than those of the control group[(81.26±8.67)points,(82.38±8.54)points,(80.18±8.05)points],and the differences were statistically significant(P<0.05);After teaching,the scores of the teaching effect of the standardized training physicians in the study group were higher than those in the control group,and the difference were statistically significant(P<0.001).After teaching,the satisfaction rate of resident physicians in the research group(94.44%)was higher than that in the control group(75.00%),and the difference was statistically significant(P<0.05).Conclusion The modified TBL teaching method,combined with Mini-CEX,can enhance the assessment scores,teaching effectiveness,and satisfaction of resident phy
