硅胶义乳对单侧乳房切除术后患者身体姿势及体像的影响

摘　　要：目的 探讨佩戴硅胶义乳对单侧乳房切除术后患者身体姿势及体像的影响。方法 采用便利抽样法,选择2020年9月至2021年3月在我院乳腺外科换药室行最后1次伤口拆线的符合纳入、排除标准并知情同意的240例单侧乳腺癌患者作为研究对象,随机分为干预组和常规组。于伤口拆线后1周,常规组佩戴传统棉质义乳,干预组佩戴硅胶义乳,在干预前、干预第6个月运用体态学和触诊学原理测量患者的身体姿势参数指标,并填写乳腺癌患者体像问卷。结果 干预后干预组头前伸(t=3.80,P<0.05)、高低肩(t=2.38,P<0.05)及肩胛骨倾斜(t=3.02,P<0.05)水平均低于常规组,且干预组干预前后的变化明显低于常规组。干预后干预组体像自评量表得分为(68.33±14.04)分、常规组为(79.99±13.68)分,两组比较差异有统计学意义(t=-6.28,P<0.05)。结论 佩戴硅胶义乳可以降低单侧乳腺癌患者体像困扰,改善单侧乳腺癌患者头前伸、高低肩、肩胛骨倾斜。但对患者短期内肩胛骨内收/外展的身体姿势没有明显改善,有待进一步研究。Objective To investigate the effect of wearing external breast prosthesis on body posture and body image of patients after unilateral mastectomy.Methods By employing the convenient sampling method,240 unilateral breast cancer patients who met the inclusion and exclusion criteria and informed consent of the last wound suture removal in the breast surgery dressing room of our hospital from September 2020 to March 2021 were selected as the study subjects,and were randomly divided into the intervention group and the regular group.One week after the wound suture was removed,the control group wore traditional cotton artificial breast,and the observation group wore silicone artificial breast.Before the intervention,and at the sixth month of the intervention,the patient's body posture parameters were measured using the principles of somatology and palpation,and the breast cancer patient's body image questionnaire was collected.Results After intervention,the levels of head extension(t=3.80,P<0.05),high and low shoulders(t=2.38,P<0.05)and scapular tilt(t=3.02,P<0.05)in the observation group were lower than those in the control group,and the changes before and after intervention in the observation group were significantly lower than those in the control group.After the intervention,the score of body image self-assessment scale in the observation group was(68.33±14.04)points,and that in the control group was(79.99±13.68)points.The difference between the two groups was statistically significant(t=-6.28,P<0.05).Conclusion Wearing silicone artificial breast can reduce the body image trouble of unilateral breast cancer patients,and improve the head extension,high and low shoulders,and scapular tilt of unilateral breast cancer patients.However,the body posture of scapula adduction/abduction has not been improved significantly in the short term,which needs further study.
