摘　　要：目的基于属性层次模型(AHM)建立特瑞普利单抗注射液的合理用药评价方案,分析该药在抗肿瘤患者中的应用情况。方法以特瑞普利单抗注射液药品说明书、相关指南等为依据,制定特瑞普利单抗注射液评价标准,采用AHM对2024年1-6月抽取的72份温州市中心医院使用特瑞普利单抗注射液的患者病历进行评价,以病历得分(MRS)为评价指标。结果MRS≥90分8例(11.11%),80分≤MRS<90分37例(51.39%),60分≤MRS<80分18例(25.00%),MRS<60分9例(12.50%)。特瑞普利单抗注射液的主要问题为用法用量及疗程不适宜(38例,52.78%),化疗联合方案不合理(27例,37.50%)。结论基于AHM对特瑞普利单抗注射液进行合理用药评价结果直观可靠,特瑞普利单抗注射液在临床使用中还须加强管理和监督,从而促进用药的合理、有效。Objective To establish a rational drug use evaluation plan for toripalimab injection based on attribute hierarchical model(AHM),and analyze the application of toripalimab in cancer patients.Methods The evaluation criteria for toripalimab injection was established based on the package inserts and relevant guidelines,and 72 medical records of patients using toripalimab injection in Wenzhou Central Hospital from January to June in 2024 were evaluated by AHM,with medical record scores(MRS)being the evaluation index.Results Totally 8 cases(11.11%)had MRS≥90,37 cases(51.39%)had 80≤MRS<90,18 cases(25.00%)had 60≤MRS<80,and 9 cases(12.50%)had MRS<60.The main issues with toripalimab injection were inappropriate dosage and course of treatment(38 cases,52.78%),and unreasonable combined chemotherapy regimen(27 cases,37.50%).Conclusion The results of rational drug use evaluation for toripalimab injection based on AHM are intuitive and reliable.It is necessary to strengthen management and supervision of toripalimab injection in clinical use to promote rational and effective drug use.
