摘　　要：为探讨牙周炎与心力衰竭发病风险之间的关联性,并为心力衰竭的预防策略提供科学依据,筛选2011—2014年美国国家健康与营养调查(NHANES)中30岁及以上成年人的数据。采用多元logistic回归模型评估牙周炎与心力衰竭发病风险的相关性,并利用公开的全基因组关联研究(GWAS)数据进行两样本孟德尔随机化(MR)分析,以探究牙周炎与心力衰竭发病风险之间的因果关联。结果显示,在NHANES数据集中,共筛选出受访者7836人,其中2282人患有牙周炎,242人患有心力衰竭。多元logistic回归分析显示,牙周炎与心力衰竭发病风险呈显著正相关(OR值=1.581,95%CI 1.062~2.351,P<0.05)。两样本MR分析进一步证实了牙周炎与心力衰竭之间的潜在因果关系(OR值=1.06,95%CI 1.01~2.74,P<0.05)。由此可见,牙周炎是心力衰竭发病的一个危险因素,早期识别和治疗牙周炎对心力衰竭的预防具有重要的临床意义。In order to explore the association between periodontitis and the risk of heart failure,and to provide a scientific basis for heart failure prevention strategies,data from adults aged 30 and above in the National Health and Nutrition Examination Survey(NHANES)from 2011 to 2014 were screened.Multiple logistic regression models were used to evaluate the correlation between periodontitis and heart failure risk,and publicly available genome-wide association studies(GWAS)data were used for two sample Mendelian randomization(MR)analysis to explore the causal association between periodontitis and heart failure risk.The results showed that a total of 7836 respondents were screened in the NHANES dataset,of whom 2282 had periodontitis and 242 had heart failure.Multiple logistic regression analysis showed a significant positive correlation between periodontitis and the risk of heart failure(OR=1.581,95%CI 1.062~2.351,P<0.05).Two sample MR analyses further confirmed the potential causal association between periodontitis and heart failure(OR=1.06,95%CI 1.01~2.74,P<0.05).This shows that periodontitis is a risk factor for the occurrence of heart failure,and early recognition and treatment of periodontitis have important clinical significance for the prevention of heart failure.
