立体定向血肿穿刺抽吸术联合醒脑静注射液治疗高血压脑出血的临床研究

摘　　要：目的探究临床高血压脑出血(HICH)治疗中醒脑静(XNJ)注射液联合立体定向血肿穿刺抽吸术(SHPS)的疗效。方法本文从2022年8月起选取HICH患者,并于2024年8月终止,共入选60例,其中采取小骨窗开颅血肿清除术(CHR)治疗的30例设为CHR组,采取XNJ注射液联合SHPS治疗的30例设为SHPS组。对比手术情况、预后量表(GCS)评分变化、并发症及自理水平。结果与CHR组相比,SHPS组的血肿清除率>80%的占比更高,手术用时更短(P<0.05)。两组HICH患者术后的GCS得分较术前均提高,且SHPS组的GCS得分更高(P<0.05)。与CHR组相比,SHPS组的并发症更少(P<0.05)。与CHR组相比,SHPS组的自理水平更高(P<0.05)。结论临床HICH治疗中XNJ注射液联合SHPS的疗效更优,可有效清除血肿,减少手术用时及并发症,还可缓解昏迷症状,促进自理能力恢复,可推广和借鉴。Objective To investigate the efficacy of XNJ injection combined with stereotactic hematoma puncture and aspiration(SHPS)in the treatment of hypertensive cerebral hemorrhage(HICH).Method In this paper,HICH patients were selected from August 2022 and terminated in August 2024,and a total of 60 cases were enrolled,of which 30 cases treated with craniectomy hematoma removal with small bone window(CHR)were set as the CHR group,and 30 cases treated with XNJ injection combined with SHPS were set as the SHPS group.The surgical conditions,changes in prognostic scale(GCS)scores,complications and self-care levels were compared.Result Compared with the CHR group,the SHPS group had a higher percentage of hematoma clearance>80%and shorter operation time(P<0.05).The postoperative GCS scores of HICH patients in both groups were improved compared with the preoperative period,and the GCS scores were higher in the SHPS group(P<0.05).There were fewer complications in the SHPS group compared with the CHR group(P<0.05).The SHPS group had a higher level of self-care compared with the CHR group(P<0.05).Conclusion The efficacy of XNJ injection combined with SHPS in the treatment of clinical HICH is better,which can effectively remove hematoma,reduce the time of surgery and complications,and also relieve the symptoms of coma and promote the recovery of self-care ability,which can be popularized and learned from.
