摘  要:目的:构建翼状胬肉切除术后干眼发生风险模型。方法:选取2020年3月—2023年1月样本医院收治的218例拟行翼状胬肉切除术患者作为研究对象。术后随访12个月,根据患者翼状胬肉切除术后是否发生干眼,分为发生组与非发生组。构建并验证翼状胬肉切除术后干眼发生风险模型。结果:随访12个月,218例患者均未失访,发生干眼48例,发生率为22.02%。发生组年龄、巩膜裸露面积、单日视觉显示终端使用时间≥2 h占比高于非发生组,日间户外活动时间长于非发生组,泪膜破裂时间(BUT)短于非发生组,差异均有统计学意义(t=5.720、10.332;χ^(2)=20.687;t=6.252、3.161,P<0.05)。logistic回归分析结果显示,年龄、巩膜裸露面积、单日视觉显示终端使用时间、BUT时间是翼状胬肉切除术后干眼发生的影响因素(OR=4.749、5.339、4.225、0.260,P<0.05)。列线图模型预测翼状胬肉切除术后干眼发生的灵敏度为91.67%,特异度为90.59%,曲线下面积(AUC)为0.913。结论:年龄较高、巩膜裸露面积较大、单日视觉显示终端使用时间较长、BUT时间较短与翼状胬肉切除术后干眼发生风险有关,构建列线图模型有助于早期甄别翼状胬肉切除术后干眼发生风险。Objective:To establish and validate the risk model of xerophthalmia after pterygium excision.Methods:218 patients with pterygium resection from March 2020 to January 2023 were selected and followed up for 12 months.According to the occurrence of dry eye after pterygium resection,they were divided into occurrence group and non-occurrence group.The risk model of dry eye after pterygium excision was constructed and validated.Results:After 12 months of follow-up,218 patients were not lost to follow-up,and 48 patients developed dry eye(22.02%).The proportion of age,sclera exposed area,daily visual display terminal use time≥2 h in the occurrence group was higher than that in the non-occurrence group,the daytime outdoor activity time was longer than that in the non-occurrence group,BUT time was shorter than that in the non-occurrence group,the differences were statistically significant(t=5.720,10.332;χ^(2)=20.687;t=6.252,3.161;P<0.05).Logistic regression analysis showed that age,exposed sclera area,single-day visual display terminal use time and BUT time were the influencing factors for the occurrence of dry eye after pterygium resection(OR=4.749,5.339,4.225,0.260;P<0.05).The sensitivity,specificity and AUC of the nomogram model for predicting the occurrence of dry eye after pterygium resection were 91.67%,90.59%and 0.913.Conclusion:Older age,larger sclera exposed area,longer BUT shorter duration of single-day visual display terminal are associated with the risk of dry eye after pterygium excision.The construction of a nomogram model is helpful for early screening of the risk of dry eye after pterygium excision.
