丁苯酞软胶囊联合多奈派齐片治疗卒中后认知障碍临床观察研究

摘　　要：目的:观察丁苯酞软胶囊联合多奈派齐片治疗卒中后认知障碍(post-stroke cognitive impairment, PSCI)患者的临床疗效。方法:选取60例PSCI患者,采用随机数字表法分为治疗组和对照组,两组各30例患者,对照组给予口服多奈派齐片,每晚1片饭后1粒(10 mg),治疗组在对照组基础上给予口服丁苯酞软胶囊,三餐前服用2粒,每粒0.2 g,所有患者均治疗4周。治疗前后给予简明精神状态量表(MMSE)、蒙特利尔认知测试(MoCA)、改良Barthel指数(MBI)、脑源性神经营养因子(BDNF)水平测评,记录、整理数据并进行统计分析。结果:治疗后,2组患者MMSE、MoCA、MBI评分均高于治疗前,治疗组评分高于对照组,差异均有统计学意义(P<0.05);治疗后对照组患者BDNF水平较治疗前无统计学差异,治疗组患者BDNF水平较治疗前及对照组患者BDNF水平均有显著提高,差异有统计学意义(P<0.05)。结论:丁苯酞软胶囊联合多奈派齐片治疗PSCI优于多奈哌齐片单药,联合用药可更快改善PSCI患者的认知功能,提高日常生活能力,值得临床进一步推荐。Objective:To observe the clinical effect of butylphthalide soft capsule combined with donepezil tablets in the treatment of patients with post-stroke cognitive impairment(PSCI).Methods:Sixty patients with PSCI were randomly divided into treatment group and control group,30 patients in each group.The control group was given oral donepezil tablets,one tablet after meals(10 mg)every night,and the treatment group was given oral butylphthalide soft capsules on the basis of the control group,taking 2 capsules before meals,0.2 g each.All patients were treated for 4 weeks.The levels of mini-mental state examination(MMSE),Montreal cognitive test(MoCA),modified Barthel index(MBI)and brain-derived neurotrophic factor(BDNF)were measured before and after treatment.The data were recorded and analyzed.Results:After treatment,the MMSE,MoCA and MBI scores of the two groups were higher than those before treatment,and the scores of the treatment group were higher than those of the control group,the differences were statistically significant(P<0.05).After treatment,there was no significant difference in the BDNF level of the control group compared with that before treatment,while the BDNF level of the treatment group was significantly higher than that before treatment and that of the control group(P<0.05).Conclusion:Butylphthalide soft capsule combined with donepezil tablets is superior to donepezil tablets alone in the treatment of PSCI.The combined treatment can improve the cognitive function and daily living ability of PSCI patients faster,which is worthy of further clinical recommendation.
