消癌平注射液联合化疗方案对中晚期食管癌患者生存率及血清相关细胞因子水平的影响

摘　　要：目的观察消癌平注射液联合多西他赛+顺铂化疗方案治疗中晚期食管癌(EC)患者的效果。方法收集郑州大学第二附属医院2020年7月至2022年12月146例中晚期EC患者临床资料行回顾性研究,依照治疗方案差异分为两组,对照组(72例)接受多西他赛+顺铂化疗方案治疗,观察组(74例)接受消癌平注射液联合多西他赛+顺铂化疗方案治疗,比较两组疾病控制率,治疗前后肿瘤标志物水平[血清癌胚抗原(CEA)、肿瘤特异性生长因子(TSGF)、细胞角质蛋白19片段(CYFRA21-1)、糖类抗原199(CA199)],免疫功能(CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)),血清相关细胞因子水平[胰岛素样生长因子1(IGF-1)、可溶性白细胞介素-2受体(sIL-2R)、转化生长因子β1(TGF-β1)],1年生存率。结果治疗后,与对照组疾病控制率比较,观察组较高(P<0.05);两组血清CEA、TSGF、CYFRA21-1、CA199水平均降低,且观察组低于对照组(P<0.05);与对照组比较,观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均较高(P<0.05);观察组血清IGF-1、sIL-2R、TGF-β1水平均低于对照组(P<0.05);观察组1年生存率较对照组高(P<0.05);观察组胃肠道反应、骨髓抑制发生率较对照组低(P<0.05),组间肝肾功能损伤、神经系统损伤发生率差异无统计学意义(P>0.05)。结论消癌平注射液联合多西他赛+顺铂化疗方案治疗中晚期EC,能降低肿瘤标志物、血清IGF-1、sIL-2R、TGF-β1表达水平,有效控制病情进展,提高患者免疫功能,延长生存期限,且能减少胃肠道反应、骨髓抑制发生。Objective To observe the effect of xiaoaiping injection combined with docetaxel+cisplatin chemotherapy regimen in the treatment of advanced esophageal cancer(EC)patients.Methods The clinical data of 146 patients with advanced EC in the Second Affiliated Hospital of Zhengzhou University from July 2020 to December 2022 were collected for retrospective study.The patients were divided into two groups according to the difference in treatment plan.The control group(72 cases)received docetaxel+cisplatin chemotherapy,while the observation group(74 cases)received xiaoaiping injection combined with docetaxel+cisplatin chemotherapy.The disease control rate was compared of two groups,as well as the tumor marker levels[serum carcinoembryonic antigen(CEA),tumor specific growth factor(TSGF),cytokeratin 19 fragment(CYFRA21-1),carbohydrate antigen 199(CA199)],immune function(CD3^(+),+CD4^(+) and CD4^(+)/CD8^(+)),serum levels of relevant cytokines[insulin-like growth factor 1(IGF-1),soluble interleukin-2 receptor(sIL-2R),transforming growth factorβ1(TGF-β1)],1-year survival rate before and after treatment.Results Compared with the disease control rate of the control group,the observation group was higher(P<0.05).After treatment,the levels of serum CEA,TSGF,CYFRA21-1,and CA199 in both groups were decreased,and the observation group were lower than the control group(P<0.05);compared with the control group,the levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in the observation group were higher(P<0.05);the levels of serum IGF-1,sIL-2R,and TGF-β1 in the observation group were lower than those in the control group(P<0.05).The 1-year survival rate of the observation group was higher than that of the control group(P<0.05).The incidence of gastrointestinal reactions and bone marrow suppression in the observation group were lower than those in the control group(P<0.05).There were no significant differences in the incidence of liver and kidney function damage and neurological damage between the two groups(P>0.05).Conclusion The combination o
