摘 要:目的探讨浆细胞CD31、CD45表达水平与初诊多发性骨髓瘤的相关性。方法选取2019年8月至2021年8月在盐城市第三人民医院初诊为多发性骨髓瘤的患者32例为病例组,同期选取12例骨髓浆细胞增多患者为对照组。用流式细胞技术检测两组患者浆细胞表面分化抗原,分析浆细胞CD31和CD45的表达差异。同时分析病例组患者CD31和CD45与原始浆细胞比例的相关性。结果与对照组相比,病例组患者浆细胞CD31、CD45的表达率均明显降低,差异具有统计学意义[%:(65.37±3.93)比(98.38±4.34),(1.28±0.07)比(12.34±1.03),t值分别为24.12、37.17,P值均<0.05]。流式细胞结果显示,17例多发性骨髓瘤患者浆细胞集中在流式细胞图上CD31/CD45^(+)区,15例多发性骨髓瘤患者浆细胞均集中在流式细胞图上CD31/CD45^(-)区。相关分析显示,骨髓瘤细胞表面CD31与CD45的表达比例与原始浆细胞的比例呈负相关,骨髓瘤细胞表面CD31与CD45表达呈正相关(r=-0.85、-0.86、0.85,P值均<0.05)。结论骨髓浆细胞的恶性病变有可能导致CD31和CD45浆细胞表达下降,CD31、CD45表达率与初发多发性骨髓瘤患者病情分期呈负正相关。Objective To explore the correlation between the expression levels of CD31 and CD45 in plasma cells and newly diagnosed multiple myeloma.Methods From August 2019 to August 2021,32 patients with multiple myeloma diagnosed for the first time in Yancheng Third People's Hospital were selected as the case group,and 12 patients with myeloplasmacytosis(normal cell morphology)were selected as the control group.The flow cytometry was used to detect the antigens differentiated on the surface of plasma cells,and the expression of CD31 and CD45 in plasma cells were analyzed in two groups of patients.At the same time,the correlation between CD31 and CD45 and the proportion of original plasma cells was analyzed in the case group patients.Results Compared with the control group,the expression rates of CD31 and CD45 in plasma cells of patients in the case group were significantly lower,with statistical significance[%:(65.37±3.93)vs(98.38±4.34),(1.28±0.07)vs(12.34±1.03),t values were 24.12,37.17,both P values<0.05].The results of flow cytometry showed that the plasma cells of 17 patients with multiple myeloma were concentrated in the CD31/CD45^(+)region on the flow cytometry,and the plasma cells of 15 patients with multiple myeloma were concentrated in the CD31/1CD45^(-)region on the flow cytometry.Correlation analysis showed that the expression ratio of CD31 and CD45 on the surface of myeloma cells was negatively correlated with the ratio of original plasma cells,while the expression ratio of CD31 and CD45 on the surface of myeloma cells was positively correlated(r=-0.85,-0.86,0.85,all P values<0.05).Conclusion Malignant lesions of bone marrow plasma cells may lead to a decrease in the expression of CD31 and CD45 plasma cells,and the expression rates of CD31 and CD45 are negatively and positively correlated with the stage of initial multiple myeloma patients.
