MRI动态增强扫描在腮腺混合瘤诊断中的应用及影像特点分析

摘　　要：目的:分析MRI动态增强扫描在腮腺混合瘤诊断中的应用价值及影像特点。方法:选取经病理证实的44例腮腺良性肿瘤患者,其中腮腺混合瘤24例(腮腺混合瘤组)与腮腺腺淋巴瘤20例(腺淋巴瘤组)。比较2组MRI动态增强扫描TIC类型及相关参数[血管渗透性转运常数(K^(trans))、血管外细胞外间隙容积分数(V_e)与速率常数(K_(ep))]。绘制ROC曲线分析MRI动态增强扫描对腮腺混合瘤的诊断效能。结果:腮腺混合瘤的MRI表现以边界清晰、形状规则、T_1WI低信号、T_2WI高信号、增强扫描病灶渐进性强化为主。腮腺混合瘤组持续型TIC占比显著高于腺淋巴瘤组(P<0.05)。腮腺混合瘤组K^(trans)、K_(ep)显著低于腺淋巴瘤组(均P<0.05)。2组V_e比较,差异无统计学意义(P>0.05)。经ROC曲线分析发现,TIC类型、K^(trans)、K_(ep)及三者联合诊断腮腺混合瘤AUC分别为0.896、0.670、0.885、0.981,三者联合AUC最大,诊断敏感度91.70%,特异度100.00%。结论:MRI动态增强扫描对腮腺混合瘤具有较高诊断效能。Objective:To analyze the application value of dynamic contrast-enhanced MRI(DCE-MRI)in the diagnosis of mixed tumor of parotid gland and its imaging characteristics.Methods:This study included 44 patients with histopathologically confirmed benign parotid tumors(24 cases of mixed tumor of paroid gland,20 cases of parotid gland lymphoma).DCE-MRI parameters including TIC patterns,K^(trans),Ve,K_(ep) were compared between the two groups.ROC curve was used to assess the diagnostic efficacy.Results:MRI findings of mixed tumor included clear boundaries,regular shapes,low signal on T1WI,high signal on T2WI,and progressive enhancement.The proportion of persistent TIC pattern in the mixed tumor was significantly higher than that in the parotid gland lymphoma(P<0.05).K^(trans) and K_(ep) in the mixed tumor were significantly lower than those in the parotid gland lymphoma(both P<0.05).There was no significant difference in Ve between the two groups(P>0.05).ROC curve analysis revealed AUC values of 0.896 for TIC pattern,of 0.670 for K^(trans),of 0.885 for K_(ep),and of 0.981 for the combined three parameters,respectively.The AUC value of the combined three parameters was the highest,with the sensitivity of 91.70%,the specificity of 100.00%.Conclusion:DCE-MRI has a high diagnostic efficacy for the mixed tumor of parotid gland.
