摘 要:目的:探讨3D-T_(1)WI、3D-T_(2) FLAIR、3D-BrainVIEW-T_(1)黑血增强MRI(CE-MRI)对脑转移瘤的检出能力。方法:收集经病理确诊或经临床随访证实的25例脑转移瘤患者,同时行3D-T_(1)WI、3D-T_(2) FLAIR和3D-BrainVIEW-T_(1)黑血序列增强扫描。按病灶直径分为3组(≤3 mm、>3~<10 mm、≥10 mm),比较以上3个序列对脑转移瘤的检出数目及对不同直径病灶的检出率。结果:25例共检出246个脑转移瘤病灶,3D-T_(1)WI、3D-T_(2) FLAIR和3D-BrainVIEW-T_(1)黑血增强扫描对脑转移瘤的检出数目分别为153、183和246个。两两比较发现,3D-BrainVIEW-T_(1)黑血序列对病灶的检出数目显著高于3D-T_(1)WI和3D-T_(2) FLAIR(P=0.003,P=0.002);3种序列对直径≤3 mm及>3~<10 mm、≥10 mm病灶检出情况差异有统计学意义(P=0.034)。结论:相较于3D-T_(2)FLAIR和3D-T_(1)WI序列,3D-BrainVIEW-T_(1)黑血增强扫描对脑转移瘤的检出率更高,尤其是对微小转移瘤的检出更有价值,临床常规应用可早期发现转移病灶。Objective:To investigate the ability of 3D-T_(1)WI,3D-T_(2) FLAIR and 3D-BrainVIEW-T_(1)WI black blood contrast enhancement sequences in the detection of brain metastases.Methods:A total of 25 patients clinically or pathologically diagnosed as brain metastases were enrolled.All patients underwent 3D-T_(1)WI,3D-T_(2) FLAIR,and 3D-BrainVIEW-T_(1)WI black blood sequences.According to the detected lesions’diameter,the brain metastases were divided into three groups(≤3 mm,3—10 mm,≥10 mm).The number of brain metastases detected by the above three sequences and the detection rate of lesions with different diameters were compared.Results:A total of 246 brain metastases were detected in 25 cases.The numbers of metastases detected by 3D-T_(1)WI,3D-T_(2) FLAIR and 3D-BrainVIEW-T_(1)WI black blood sequences were 153,183 and 246,respectively.Pairwise comparisons revealed that the 3D-BrainVIEW-T_(1)WI black blood sequence detected more lesions than 3D-T_(1)WI and 3D-T_(2) FLAIR sequences(P=0.003,P=0.002).Additionally,three sequences demonstrated significant difference in the detection rate of three type of metastases with different diameter(P=0.034).Conclusions:The 3D-BrainVIEW-T_(1)WI black blood contrast enhancement sequence exhibits a higher detection rate for brain metastases compared to 3D-T_(1)WI and 3D-T_(2) FLAIR sequences,particularly for small metastases.Its routine clinical application is recommended to facilitate early detection of metastatic lesions.
