根尖周炎对上颌窦黏膜厚度影响的锥形束CT分析

摘　　要：目的运用锥形束CT研究慢性根尖周炎对上颌窦黏膜厚度的影响,探讨黏膜厚度的影响因素。方法回顾性分析2022年9月至2023年8月于首都医科大学附属北京安贞医院口腔科就诊患者的CBCT图像,测量根尖病变(AP)组和正常(NAP)组上颌窦黏膜厚度,以及根尖周病变大小、病变边缘与窦底距离、根尖至窦底距离等指标,分析上颌窦黏膜增厚的影响因素。结果AP组黏膜平均厚度为(4.47±3.22)mm,高于NAP组(1.87±2.76)mm(P<0.05);上颌窦黏膜厚度和年龄呈弱负相关(r=-0.17,P<0.05);和根尖到窦底距离相关性极弱(r=0.03,P<0.05);和病变直径呈弱正相关(r=0.18,P<0.05);和病变边缘到窦底距离呈中负相关(r=-0.54,P<0.05)。结论上颌窦炎的严重程度可能更大程度上取决于病变边缘到窦底的距离,边缘距上颌窦越近,黏膜厚度受根尖周炎影响越大。Objective The present study aimed to analyze the effect of chronic apical periodontitis(AP)on maxillary sinus mucosal thickening by CBCT and the influencing factors of mucosal thickening.Methods This study retrospectively reviewed patients with and without chronic apical periodontitis were admitted to the Department of Stomatology of Beijing Anzhen Hospital of Capital Medical University from September 2022 to August 2023.The relationship between the floor of the maxillary sinus and the apical and the lesion edge was evaluated while the maxillary sinus mucosal thickness,periapical lesion size were observed.Then the influencing factors of maxillary sinus mucosal thickening were analyzed.Results The mean mucosal thickness in the AP group was(4.47±3.22)mm,which was higher than that in the control group(1.87±2.76)mm(P<0.05).There was a weak negative correlation between mucosal thickness and age(r=-0.17,P<0.05),an extremely weak correlation with the distance from the apex to the sinus floor(r=0.03,P<0.05),a weak positive correlation with lesion diameter(r=0.18,P<0.05),and a moderate negative correlation with the distance from the lesion margin to the sinus floor(r=-0.54,P<0.05).Conclusions The severity of maxillary sinusitis may be more dependent on the distance of the periapical inflammatory margin from the sinus floor rather than the apical position and the size of lesion.
