摘 要:目的:探究下肢康复机器人与动态等速肌力训练系统对胸腰段脊髓损伤患者康复效果的影响。方法:选取2020年4月—2024年4月陕西省康复医院收治的100例胸腰段脊髓损伤患者作为研究对象,按照随机数表法将其分为研究组(50例,下肢康复机器人与动态等速肌力训练)和对照组(50例,常规护理)。两组患者均治疗3个月,比较其美国脊髓损伤协会(ASIA)分级、平衡能力、综合活动功能能力、下肢肌力及下肢步行能力的差异。结果:相较于治疗前,治疗3个月后两组ASIA分级均改善,且研究组优于对照组(P<0.05)。两组脊髓功能独立性(SCIM)评分和Berg平衡量表(BBS)评分的时点、组间、交互比较,差异有统计学意义(P<0.05),相较于治疗前,两组患者治疗1个月和3个月后SCIM评分和BBS评分均升高,且研究组高于对照组(P<0.05)。相较于治疗前,两组患者治疗3个月后伸肌群和屈肌群的峰力矩及力矩加速能、腘绳肌和股四头肌肌力比例、臀大肌肌力均升高,且研究组高于对照组(P<0.05)。相较于治疗前,两组患者治疗3个月后脊髓损伤步行指数(WISCI)Ⅱ和6 min步行测试结果均提升,10 m步行测试用时均降低,且研究组均优于对照组(P<0.05)。结论:下肢康复机器人与动态等速肌力训练系统应用于胸腰段脊髓损伤患者,对其康复具有较好的效果,可有效改善ASIA脊髓损伤,提高下肢肌力与下肢步行能力。Objective:To explore the effect of applying lower limb rehabilitation robot combined with dynamic isokinetic muscle strength training system in the rehabilitation of patients with thoracolumbar spinal cord injury.Methods:100 patients with thoracolumbar spinal cord injury Who were treated in Shaanxi KangFu Hospital from April 2020 to April 2024 were selected.They were divided into the study group(lower limb rehabilitation robot combined with dynamic isokinetic muscle strength training system,n=50)and the control group(conventional nursing,n=50)using a random number table.Both groups were treated for 3 months,and the differences in ASIA spinal cord injury grading,balance ability,comprehensive motor function ability,lower limb muscle strength,and walking ability were compared.Results:Compared with that before treatment,the ASIA grading in both groups was improved after 3 months of treatment,and it was better in the study group than that in the control group(P<0.05).There were significant differences in the time,inter-group,and interaction of SCIM scores and BBS scores between the two groups(P<0.05).Compared with that before treatment,the SCIM scores and BBS scores of the two groups were increased after 1 month and 3 months of treatment,and they were higher in the study group than those in the control group(P<0.05).The peak moment and moment acceleration energy of extensor and flexor muscles,the proportion of hamstring and quadriceps muscle strength,and gluteus maximus muscle strength were increased in both groups after 3 months of treatment,and they were higher in the study group than those in the control group(P<0.05).The results of the WISCIⅡand 6-min walk test were elevated,and the 10-m walking time was reduced in both groups after 3 months of treatment,and the they were better in study group than those in the control group(P<0.05).Conclusion:The application of lower limb rehabilitation robot combined with dynamic isokinetic muscle strength training system in patients with thoracolumbar spinal cord injury has a
