玻璃体腔内硅油填充时间对孔源性视网膜脱离患者眼轴、前房深度等眼部参数的影响

摘　　要：目的探讨玻璃体切割术后不同的硅油填充时间对孔源性视网膜脱离患者眼部参数的影响。方法选取2022年6月至2024年6月在常熟市第一人民医院眼科就诊的孔源性视网膜脱离患者38例(38只眼)进行回顾性研究,根据硅油填充时间长短分为长期填充组(20只眼)和短期填充组(18只眼)。所有患者均行23G玻璃体切割+激光封闭裂孔+硅油填充术。两组患者的硅油填充时间分别为4.85±1.29个月和2.93±0.46个月。硅油取出术后进行1个月的随访。观察指标包括眼轴、前房深度、晶体厚度以及玻璃体长度,采用IOL-master测量记录。结果长期填充组的患者取油前以及取油术后1个月的眼轴和玻璃体长度明显增加。短期填充组仅有取油术后一月的眼轴明显增加。长期填充组在取油术前和取油后1月的眼轴和玻璃体长度均大于短期填充组。结论较长的硅油填充时间对于孔源性视网膜脱离患者术后的眼轴以及玻璃体长度影响较大。ObjectiveTo investigate the effect of different flling time of silicone oil on ocular parameters in patients with rheogenous retinal detachment.Methods A retrospective study was conducted on 38 patients(38 eyes)with rhegmatogenous retinal detachment who underwent surgical treatment in the First People's Hospital of Changshu from June 2022 to June 2024.According to the length of silicone oil tamponade,they were divided into long-term tamponade group(20 eyes)and short-term tamponade group(18eyes).All patients underwent 23G vitrectomy,laser photocoagulation and silicone oil tamponade.The silicone oil filling time of the two groups was 4.85±1.29 months and 2.93±0.46 months,respectively.The patients were followed up for 1 month after silicone oil removal.The axial length,anterior chamber depth,lens thickness and vitreous length were measured and recorded by IOL-master.ResultsPatients in the long-term tamponade group had a significant increase in axial length and vitreous lengh before and 1 month afer oil removal.In the short-term tamponade group.only the axial length at 1 month after oil removal was significantly increased.The axial length and vitreous length of the long-term tamponade group were larger than those of the short-term tamponade group before and 1 month after oil removal.Conclusion Long flling time of silicone oil has great influence on the ocular axis and vitreous length of patients with rheogenic retinal detachment.
