超声乳化联合小梁切除术治疗闭角型青光眼并白内障患者的效果及其对视力、房角结构的影响

摘　　要：目的观察小梁切除术联合超声乳化治疗闭角型青光眼并白内障的效果及其对房角结构、视力的影响。方法随机数字表法将我科97例闭角型青光眼并白内障患者分组,单一组48例接受小梁切除术,联合组49例实施小梁切除术联合超声乳化治疗,观察患者并发症情况,术后1个月比较两组视功能、房角结构参数、眼压和视觉质量。结果联合组1个月后调制传递函数截止频率(0.16±0.04)mm、斯特列尔比(20.18±2.12)中央前房深度(2.60±0.44)mm、最佳矫正视力(0.66±0.06)、房角开放距离(0.24±0.06)度、小梁虹膜角(19.88±2.93)mm高于单一组的(0.14±0.05)mm、(19.24±1.96)、(2.21±0.33)mm、(0.49±0.08)(0.18±0.04)度、(15.29±2.81)mm,(t=2.1777,2.2663,4.9307,11.8215,5.8058,7.8718,P<0.05),联合组1个月后客观散射指数(3.45±0.35)mm、散光度(0.70±0.06)D、I0P(17.19±2.18)mmHg及并发症发生率10.20%(5/49)低于单一组的(3.68±0.41)mm、(0.86±0.08)D、(21.14±3.55)mmHg及27.08%(13/48),(t=2.9736,11.1261,6.6187,χ^(2)=4.5710,均P<0.05),小梁睫状突距离与单一组无统计学差异(P>0.05)。结论小梁切除术联合超声乳化应用于闭角型青光眼并白内障可安全降低患者眼压,改善房角结构和视功能,促进视觉质量的提升。Objective To observe the effect of trabeculectomy combined with phacoemulsification in the treatment of angle-closure glaucoma and cataract,and its influence on the Angle structure and visual acuity.Methods 97 patients with angle-closure glaucoma and cataract were divided into two groups by random number table method.48 patients in the single group received trabeculectomy,and 49 patients in the combined group received trabeculectomy combined with phacoemulsification.The complications of the patients were observed.Results After 1 month in the combined group,the modulation transfer function cut off frequency(0.16±0.04)mm,strehl ratio(20.18±2.12),central anterior chamber depth(2.60±0.44)mm,best corrected visual acuity(0.66±0.06),roomAngleopening distance(0.24±0.06)degre,trabecular iris Angle(19.88±2.93)mm was higher than that of single group(0.14±0.05)mm,(19.24±1.96),(2.21±0.33)mm,(0.49±0.08),(0.18±0.04),(15.29±2.81)mm,(t=2.1777,2.2663,4.9307,11.8215,5.8058,7.8718,P<0.05),The objective scattering index(3.45±0.35)mm,difuser(0.70±0.06)D,I0P(17.19±2.18)mmHgandcomplication rate of 10.20%(5/49)in the combined group were lower than those in the single group(3.68±0.41)mm,(0.86±0.08)D,(21.14±3.55)mmHg and 27.08%(13/48)(t=2.9736,11.1261,6.6187,χ^(2)=4.5710,llP<0.05),therewasno significant differencein trabecular ciliary process distance between the two groups(P>0.05).Conclusion Trabeculectomy combined with phacoemulsification for angle-closure glaucoma and cataract can safely reduce the IOP,improve the angular structure and visual function,and promote the improvement of visual quality.
