白内障超声乳化吸除联合小梁切开术治疗闭角型青光眼的临床观察

摘　　要：目的探讨合并白内障的闭角型青光眼行白内障超声乳化吸除房角分离及小梁切开术手术效果。方法对来我院就诊的116例合并有白内障的闭角型青光眼,选取眼压在25mmHg到40mmHg之间的病例,行白内障超声乳化吸除+人工晶体植入术、房角分离、小梁切开术,比较术前与术后1个月视力、前房深度、眼压及术后并发症的发生情况。结果116只眼的术后裸眼视力均有不同程度的提高(100%,116/116),术前、术后裸眼视力差异有统计学意义(Z=-5.987,P<0.05)。中央前房深度由术前的(1.79±0.46)mm,增加到术后的(2.41±0.37)mm,两者差异有统计学意义(t=7.928,P<0.05)。术后眼压均在(10~21mmHg)之间。术中及术后均未发生后囊破裂,角膜水肿等并发症。随访1年,无一例发生青光眼。结论对于合并白内障的闭角青光眼行白内障超声乳化吸除人工晶体植入术、房角分离、小梁切开术对防治青光眼的发生,改善视功能有重要的意义,而且手术简单,并发症少,值得推广。ObjectiveTo investigate the effects of phacoemulsification aspiration,goniosynechialysis and trabeculotomy in patients with angle-closure glaucoma complicated with cataract.Methods A total of 116 patients with angle-closure glaucoma with cataract were selected to have intraocular pressure between 25 mmHg and 40 mmHg,and phacoemulsification+intraocular lens implantation,goniosynechialysis and trabeculotomy were performed,the preoperative and 1-month postoperative visual acuity,anterior chamber depth,intraocular pressure and the occurrence of postoperative complications were compared.Results The postoperative unaided visual acuity of 116 eyes was improved to varying degrees(100%,116/116),and the difference between preoperative and postoperative unaided visual acuity was statistically significant(Z=-5.987,P<0.05).The depth of the central anterior chamber increased from(1.79±0.46)mm before surgery to(2.41±0.37)mm after operation,and the difference between the two was statistically significant(t=7.928,P<0.05).The intraocular pressure after surgery was between(10-21mmHg).There were no complications such as posterior capsule rupture and corneal edema during and after surgery.After 1 year of follow-up,there was no case of glaucoma.Conclusion For angle-closure glaucoma with cataract,phacoemulsification aspiration intraocular lens implantation,goniosynechialysis,and trabeculotomy are of great significance for the prevention and treatment of glaucoma and the improvement of visual function.
