摘　　要：目的探讨分析在乳磨牙活髓切断术中采用iRoot BP Plus和自固化磷酸钙糊剂的应用效果。方法选取乳磨牙深龋近髓或早期牙髓炎患儿96例,均采取乳磨牙活髓切断术治疗,按1∶1随机数字表法分为iRoot BP Plus组(n=48,将iRoot BP Plus作为盖髓剂)和自固化磷酸钙糊剂组(n=48,将自固化磷酸钙糊剂作为盖髓剂)。比较两组的手术成功率、术后反应、牙龈健康[牙龈指数(GI)、龈沟出血指数(SBI)]。结果两组术后3、6个月的手术成功率之间差异无统计学意义(P>0.05);两组术后12个月的手术成功率对比,iRoot BP Plus组更高(P<0.05)。两组术后3个月、6个月的牙齿松动、叩痛、窦道不完整、咬合功能异常、牙髓活力异常、牙本质异常、根尖病变发生率比较差异无统计学意义(P>0.05);iRoot BP Plus组术后12个月的反应发生率8.33%低于自固化磷酸钙糊剂组22.92%(P<0.05)。iRoot BP Plus组术后6个月、12个月的GI指数、SBI指数低于自固化磷酸钙糊剂组(P<0.05)。结论iRoot BP Plus和自固化磷酸钙糊剂在乳磨牙活髓切断术中均能达到一定的手术效果,但前者术后12个月的手术成功率更高,术后反应发生率更低,牙髓健康恢复效果更优。Objective To compare the clinical efficacy of iRoot BP Plus and self-setting calcium phosphate paste in pulpotomy of primary molars.Methods Ninety-six children with deep caries near the pulp or early pulpitis in primary molars were included.All underwent pulpotomy and were randomly divided into the iRoot BP Plus group(n=48,using iRoot BP Plus as a capping material)and the self-setting calcium phosphate paste group(n=48,using self-setting calcium phosphate paste as a capping material).Surgical success rates,postoperative reactions,and gingival health[gingival index(GI)and sulcus bleeding index(SBI)]were compared between the two groups.Results There were no significant differences in surgical success rates between the groups at 3 and 6 months postoperatively(P>0.05).However,at 12 months,the iRoot BP Plus group had a significantly higher success rate(P<0.05).At 3 and 6 months postoperatively,the incidence of complications such as tooth mobility,percussion pain,incomplete sinus tract,occlusal dysfunction,abnormal pulp vitality,dentin abnormalities,and apical lesions showed no significant differences between the groups(P>0.05).At 12 months,the iRoot BP Plus group exhibited a lower complication rate(8.33%)compared to the self-setting calcium phosphate paste group(22.92%,P<0.05).The iRoot BP Plus group had significantly lower GI and SBI scores at 6 and 12 months postoperatively compared to the self-setting calcium phosphate paste group(P<0.05).Conclusion Both iRoot BP Plus and self-setting calcium phosphate paste achieved satisfactory results in pulpotomy of primary molars.However,iRoot BP Plus demonstrated higher success rates,lower postoperative reaction rates,and superior pulp health recovery at 12 months postoperatively.
