丁苯酞氯化钠注射液联合己酮可可碱对脑小血管病认知障碍患者脑微循环及神经细胞因子的影响

摘  要:目的:探讨在脑小血管病认知障碍患者中应用丁苯酞氯化钠注射液联合己酮可可碱的效果。方法:选取苏州明基医院2023年5月—2024年4月收治的60例脑小血管病认知障碍患者,随机分为对照组(n=30,予以丁苯酞氯化钠注射液治疗)与研究组(n=30,予以丁苯酞氯化钠注射液联合己酮可可碱治疗)。比较两组脑微循环[脑血管动态阻力(DR)、临界压力(CP)、血脑屏障通透性]、氧化应激[晚期糖基化终末产物(AGEs)及谷胱甘肽过氧化物酶(GSH-Px)、亲环蛋白A(CyPA)]、神经细胞因子[神经元特异性烯醇化酶(NSE)、神经生长因子(NGF)、脑源性神经营养因子(BDNF)]、认知状态[简易智力状态检查量表(MMSE)、蒙特利尔认知评估量表(MoCA)],并记录两组不良反应发生情况。结果:治疗后,研究组DR、血脑屏障通透性均低于对照组,CP较对照组高(P<0.05);研究组治疗后AGEs、CyPA均低于对照组,GSH-Px较对照组高(P<0.05);研究组治疗后NSE低于对照组,BDNF、NGF均较对照组高(P<0.05);研究组治疗后MMSE、MoCA评分均高于对照组(P<0.05);研究组不良反应发生率与对照组比较,差异无统计学意义(P>0.05)。结论:丁苯酞氯化钠注射液联合己酮可可碱可改善脑小血管病认知障碍患者脑部微循环,减轻氧化应激,调节神经细胞因子水平,减轻认知障碍,且用药安全性较高。Objective:To investigate the effects of Butylphthalide and Sodium Chloride Injection combined with Pentoxifylline in patients with cognitive impairment of cerebral microvascular disease.Method:A total of 60 patients with cognitive impairment of cerebral small vascular disease admitted to Suzhou Benq Hospital from May 2023 to April 2024 were selected and divided into control group(n=30,treated with Butylphthalide and Sodium Chloride Injection)and study group(n=30,treated with Butylphthalide and Sodium Chloride Injection combined with Pentoxifylline)by random number table method.Cerebral microcirculation[cerebral vascular dynamic resistance(DR),critical pressure(CP),blood-brain barrier permeability],oxidative stress[advanced glycation end products(AGEs),glutathione peroxidase(GSH-Px),cycliphilin A(CyPA)],nerve cytokines[neuron-specific enolase(NSE),nerve growth factor(NGF),brain-derived neurotrophic factor(BDNF)],cognitive status[mini-mental state examination(MMSE),Montreal cognitive assessment(MoCA)]were compared between the two groups,and the occurrence of adverse reactions in both groups were recorded.Result:After treatment,DR and blood-brain barrier permeability of the study group were lower than those of the control group,CP was higher than that of the control group(P<0.05).After treatment,AGEs and CyPA in study group were lower than those in control group,and GSH-Px was higher than that in control group(P<0.05).After treatment,NSE in the study group was lower than that in the control group,BDNF and NGF were higher than those in the control group(P<0.05).After treatment,MMSE and MoCA scores of the study group were higher than those of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the study group and the control group(P>0.05).Conclusion:Butylphthalide and Sodium Chloride Injection combined with Pentoxifylline can improve the cerebral microcirculation,reduce oxidative stress,regulate the levels of nerve cytokines and reduce cognitive impairment in pat
