激光治疗对慢性牙周炎患者口腔菌群、牙龈微循环的影响

摘　　要：目的:分析激光治疗对慢性牙周炎患者口腔菌群、牙龈微循环、咀嚼功能的影响。方法:选取2021年3月—2023年10月在兰州大学第二医院就诊的60例慢性牙周炎患者,按照随机数字表法分成对照组与研究组,各30例。对照组接受常规基础治疗,研究组则接受常规基础治疗联合激光技术治疗;比较两组的临床疗效、口腔菌群、牙龈微循环、咀嚼功能。结果:研究组临床总有效率明显高于对照组,差异有统计学意义(P<0.05)。治疗后,研究组牙周附着丧失(attachment loss,AL)、牙周袋探诊深度(probing depth,PD)、龈沟出血指数(sulcus bleeding index,SBI)、菌斑指数(plaque index,PLI)均明显低于对照组,差异均有统计学意义(P<0.05)。治疗后,研究组牙龈卟啉单胞菌(Porphyromonas gingivalis,PG)明显低于对照组,乳酸杆菌(L.bulgaricus,LB)则明显高于对照组,差异均有统计学意义(P<0.05)。治疗后,研究组牙龈血流速度(blood current velocity,BCV)、血流量(gingival blood flow,GBF)、咀嚼功能评分均明显高于对照组,差异均有统计学意义(P<0.05)。结论:慢性牙周炎患者在接受基础治疗时,联合应用激光治疗能明显提升治疗效果,不但能改善各项临床指标,而且能改善菌群失调、牙龈微循环及咀嚼功能,也不会发生显著的不良反应,安全可靠。Objective:To analyze the effects of laser therapy on oral microflora,gingival microcirculation and masticatory function in patients with chronic periodontitis.Method:A total of 60 patients with chronic periodontitis treated in the Second Hospital of Lanzhou University from March 2021 to October 2023 were selected and divided into control group and study group according to random number table method,with 30 cases in each group.The control group received conventional basic treatment,and the study group received conventional basic treatment combined with laser technology.The clinical efficacy,oral microflora,gingival microcirculation and masticatory function of the two groups were compared.Result:The total clinical effective rate of the study group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).After treatment,attachment loss(AL),probing depth(PD),sulcus bleeding index(SBI),plaque index(PLI)in the study group were significantly lower than those in control group,the differences were statistically significant(P<0.05).After treatment,Porphyromonas gingivalis(PG)in the study group was significantly lower than that in the control group,and L.bulgaricus(LB)was significantly higher than that in the control group,the differences were statistically significant(P<0.05).After treatment,blood current velocity(BCV),gingival blood flow(GBF)and masticatory function score in the study group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:When patients with chronic periodontitis receive basic treatment,the combined application of laser therapy can significantly improve the therapeutic effect,not only can improve the clinical indicators,but also can improve the flora imbalance,gingival microcirculation and mastication function,and will not occur significant adverse reactions,safe and reliable.
