经脐单孔腹腔镜在卵巢囊肿中的疗效及对胃肠功能状态的影响研究

摘　　要：目的探究及观察经脐单孔腹腔镜在卵巢囊肿中的疗效及对胃肠功能状态的影响。方法将2022年6月至2024年3月盐城市第三人民医院的80例卵巢囊肿患者根据随机数字表法分为两组。对照组的40例行传统三孔腹腔镜手术治疗,观察组的40例则应用经脐单孔腹腔镜手术治疗。比较两组术中出血量、手术时间、住院时间、胃肠功能恢复时间、术后消化系统相关并发症(恶心、呕吐及腹胀)发生率、手术前后的不适感程度(VAS)及两项胃肠激素。结果两组患者之间的术中出血量、手术时间及住院时间比较无统计学意义(P>0.05);观察组的胃肠功能恢复时间短于对照组,三项术后消化系统相关并发症发生率低于对照组(P<0.05);术前两组的VAS评分及胃肠激素比较无统计学意义(P>0.05);术后24 h及48 h观察组的VAS评分低于对照组,胃肠激素高于对照组(P<0.05)。结论经脐单孔腹腔镜对卵巢囊肿患者术后不适感的控制效果较好,有助于术后胃肠功能的恢复。Objective To investigate the effect of transumbilical single-port laparoscopy in the patients with ovarian cyst and influence for gastrointestinal function state.Methods Eighty patients with ovarian cyst from June 2022 to March 2024 were divided into two groups by the table of random number.40 cases of control group were treated with traditional three-port laparoscopic surgery,40 cases of observation group were treated with transumbilical single-port laparoscopic surgery.Then the intraoperative blood loss,operation time,hospitalization time,recovery time of gastrointestinal function,postoperative digestive system related complications(nausea,vomiting and abdominal distension)rates,discomfort degree(VAS score)and two items of gastrointestinal hormonesbefore and after the operation of two groups were compared.Results There was no significant difference in intraoperative blood loss,operation time and hospital stay between the two groups(P>0.05).The recovery time of gastrointestinal function in the observation group was shorter than that in the control group,and the three items of postoperative digestive system-related complications rate was lower than that in the control group(P<0.05).There was no statistical significance in VAS scores and gastrointestinal hormones between the two groups before surgery(P>0.05).VAS score of observation group was lower than that of control group 24 hours and 48 hour after operation,and gastrointestinal hormone was higher than that of control group(P<0.05).Conclusions The control effect of transumbilical single-port laparoscopy for postoperative discomfort is better,and it is helpful to the promoion of postoperative gastrointestinal function of patients.
