摘　　要：目的探讨经皮穴位电刺激(TEAS)对睡眠障碍老年患者下肢手术后疲劳及谵妄的影响。方法江苏省睢宁县人民医院医疗系统中104例存在睡眠障碍(术前匹兹堡睡眠质量指数PSQI>5分)并接受下肢手术的老年患者为研究对象。纳入时间2023年10月至2024年10月,依据随机数字表法分为对照组(52例)与观察组(52例),对照组实施蛛网膜下隙阻滞麻醉进行常规手术操作,观察组在对照组的基础上进行TEAS。对比两组睡眠质量、术后谵妄、术后疲劳及术后恢复指标。结果术后出院当天及术后1个月,观察组PSQI评分低于对照组(P<0.05);观察组术后Christensen疲劳评分低于对照组(P<0.05);术后1、3、5 d,观察组意识模糊评估法评分低于对照组,谵妄发生率低于对照组(P<0.05)。结论围手术期TEAS的应用,有助于改善老年下肢手术患者的睡眠障碍状况,可降低术后疲劳程度,减少术后谵妄发生。Objective To explore the effect of transcutaneous electrical acupoint stimulation(TEAS)on fatigue and delirium in elderly patients with sleep disorders after lower extremity surgery.Methods A total of 104 elderly patients with sleep disorders(preoperative Pittsburgh sleep quality index,PSQI>5)who underwent lower extremity surgery in Suining County People’s Hospital,Jiangsu Province’s medical system were selected as the study subjects.The study was conducted from October 2023 to October 2024,according to the random number table method,they were divided into the control group(52 cases)and the observation group(52 cases).The control group received subarachnoid block anesthesia for routine surgical procedures,while the observation group underwent TEAS on the basis of the control group.Compare the sleep quality,postoperative delirium,postoperative fatigue,and postoperative recovery indicators between two groups.Results On the day of discharge and one month after surgery,the PSQI score in the observation group was significantly lower compared to the control group(P<0.05).Postoperative Christensen fatigue score in the observation group was lower than that in the control group(P<0.05).On postoperative days 1,3,and 5,the observation group had lower scores in the confusion assessment method and a lower incidence of delirium compared to the control group(P<0.05).Conclusions The application of perioperative TEAS can help improve sleep disorders in elderly patients undergoing lower extremity surgery,reduce postoperative fatigue,decrease postoperative delirium.
